Overview of the Intake Template

You can use the Intake template to document the patient information.

You can document the following details of the patient in the various panels of the Intake template:

• General details of the patient
• Reason for the patient's to visit
• Patient's history
• Vital signs
• Patient's allergies
• Medications details
• Review of systems
• Orders

You can select the Record contains substance use disorder information check box to include the disclosure for Substance Abuse and Mental Health Services Administration (SAMHSA) 42 CFR Part 2 provisions in the generated documents and reports. The check box appears in the section based on the configuration of the SAMHSA CFR 42 section on the Practice Configuration template.
