Review a Patient's Symptoms for Various Systems

As part of the provider workflow, you can review and update symptoms entered during the patient's intake. OpenNextGen® Enterprise EHR. Open theSOAPtemplate, and then expand theReview of Systemspanel. To select a default set of findings, select an appropriate value in ROS Defaults. Note: You can also select Load to quickly load any saved review of systems, or select Save to save the documented review of systems to use them on other encounters. To document symptoms for multiple systems on a single template, select One Page ROS from the left navigation bar. Note: You can configure the One Page ROS link to open a gender-specific or gender-neutral template by using the Framework Content template. You can also customize the label for the One Page ROS link. To document findings for a single system, select an appropriate link from the left navigation bar. Patient Health Questionnaire (PHQ-2)

The Patient Health Questionnaire (PHQ-2) link is set up by the practice on the Practice Configuration template. The PHQ-2 is an optional setting and is a depression screening consisting of two questions. If the patient answers either of these two questions positively, a full depression screening is recommended, using the Patient Health Questionnaire (PHQ-9). If the PHQ-2 is positive, a link for the PHQ-9 automatically appears on the Depression Screening - PHQ-2 template. Document Clinical Depression Screening

You can use the PHQ-2 button to document clinical depression screening and a follow-up plan. Open NextGen® Enterprise EHR. Open the Intake template. In the Review of Systems panel, select the PHQ-2 link. The Patient Health Questionnaire (PHQ-2) window opens. Select an option for each question. To open the Patient Health Questionnaire (PHQ-9) window, select the Patient Health Questionnaire (PHQ-9) link. The Patient Health Questionnaire PHQ-2 and PHQ-9 links appear in the Review of Systems panel based on the Show PHQ Link setting done on the Practice Configuration template. Document the Review of Systems Panel

The Review of Systems panel enables you to document the findings after reviewing a list of systems. Expand the Review of Systems panel. In the Review of Systems panel, select ROS from the left pane. Note: The ROS option is dependent on the specialty selected. For Ambulatory ASC specialty, you will see GI-ROS. The speciality specific review of system template opens. To document normal values in fields of all the sections, click the ROS defaults field and select the appropriate normal default set name from the list. All of the fields in the template populate with normal values to allow for charting by exception. If applicable, select the Neg or Pos option for the applicable systems in the template. You can click the system-specific link to add detailed findings on the system. If you want to save the documented values as the default set for the current template, click the Quick Save button and enter a default set name for the template. Manage Pediatric ROS Findings

You can add or edit symptoms entered during the intake process in the Review of Systems panel on the SOAP template. Open the SOAP template. To document multiple systems on one template in the Review of Systems panel, do the following: Click the one page Pediatric ROS link in the left pane. Note: The one page Pediatric ROS link opens a gender-specific or gender-neutral ROS template based on the configuration done in the Framework Content system template. Based on your preference, you can also add a caption for the one page Pediatric ROS link in the Framework Content system template. You can document the findings of a patient's review of systems for multiple systems using a single template or a single system. Before you begin The One Page ROS option opens a gender-specific or gender-neutral Review of Systems (ROS) template based on the configuration of the Review of Systems section on the Framework Content system template. The Select other ROS option displays the practice's systems configured on the Framework Content system template. You can configure the gender-neutral Reproductive ROS template based on the configuration of the SOGI/Transgender section on the Practice Configuration template.
