Review a Patient's Symptoms for Various Systems

As part of the provider workflow, you can review and update symptoms entered during the patient's intake. OpenNextGen® Enterprise EHR. Open theSOAPtemplate, and then expand theReview of Systemspanel. To select a default set of findings, select an appropriate value in ROS Defaults. Note: You can also select Load to quickly load any saved review of systems, or select Save to save the documented review of systems to use them on other encounters. To document symptoms for multiple systems on a single template, select One Page ROS from the left navigation bar. Note: You can configure the One Page ROS link to open a gender-specific or gender-neutral template by using the Framework Content template. You can also customize the label for the One Page ROS link. Review Reasons for a Patient's Visit

You can review and update reasons for a patient's visit during the provider workflow. Open NextGen® Enterprise EHR. Open the SOAP, and then expand the Reason for Visit panel. Review and update the patient summary in the Introduction field. Review and update reasons for visit, diagnostic studies, and chronic conditions. To add any additional information, select Comments. Review Intake Comments on the SOAP Template Review comments entered during the intake if the Intake Comments link on the SOAP template indicates that content is present. Edit a Reason for Visit on SOAP You can edit existing reasons for visits on the SOAP template. Add and Review Comments for the History of Present Illness You can review and enter additional comments for a patient's history of present illness (HPI). Remove a Studies Reviewed on SOAP You can remove the historical diagnostic studies that are added to the history of patient illness in the Studies Reviewed section on the SOAP template. Document Physical Exams

To document findings for multiple physical exams on a single template, select One Page Exam from the left navigation bar. Note: You can also select Load to quickly load any saved physical exam findings, or Save to save the documented physical exam findings to use them on other encounters. To document findings for a single physical exam, select an appropriate link from the left navigation bar. To document findings for a physical exam that does not appear in the left navigation bar, select Additional. To copy physical exam findings from a previous encounter to the current encounter, select Physical Exam History.If you have copied and edited information from a previous encounter, a label appears in the Physical Exam panel and applicable documents indicating that information is copied and edited. Patient Health Questionnaire (PHQ-2)

The Patient Health Questionnaire (PHQ-2) link is set up by the practice on the Practice Configuration template. The PHQ-2 is an optional setting and is a depression screening consisting of two questions. If the patient answers either of these two questions positively, a full depression screening is recommended, using the Patient Health Questionnaire (PHQ-9). If the PHQ-2 is positive, a link for the PHQ-9 automatically appears on the Depression Screening - PHQ-2 template. Patient's Review of Systems

You can document the review findings for a patient after reviewing a list of systems. Review of Systems In Review of Systems, you can document symptoms for multiple systems on a single template with the use of one page ROS, or use the single systems in the left pane configured on the Framework Content system template. Manage Pediatric Screening

You can document and manage the screening tests and results for a patient using the screening buttons in the Screening panel on the Pediatric SOAP template. Open the Pediatric SOAP template. To document the screening instrument, click the appropriate button from the Screening panel.
