Patient's Reason for Visit
You can document a patient's reasons for visit.
In the Reason for Visit panel, you can document the History of Present Illness (HPI), problems, and diagnosis history as the reason for a patient visit.
When you select a reason for visit that is already selected in the current encounter, an alert appears indicating that the reason for visit is already selected. You can either enter the duplicate reason for visit, or remove the duplicate. For a reason for visit that has separate listings for different specialties, such as abrasion, if you select two HPIs from different specialties for the same reason, an alert appears.
Diagnostic studies that are added to the history of present illness in the Reason for Visit panel appear in the grid with Studies Reviewed in the Reason for Visit column.
In the Reason for Visit panel, you can enter the intake comments using the Intake Comments link. You can select the View All button to open the Hopi Notes template to view the HPI documentation for all chief complaints. You can use the Chronic Conditions button to open the Chronic Conditions template and document the chronic conditions for the patient.
