Document the Reason for Visit

The Reason for Visit panel is where the reason for visit, as described by the patient, is documented, along with an initial History of Present Illness (HPI). The provider should review reasons for visit, update if necessary, and complete HPI information.
Reason for Visit

  1. If the Introduction field contains text, review the content and make any necessary changes.
    The introduction appears as the first sentence of the visit note, and it can be edited for the individual patient. A typical introduction might contain the patient's age, gender, and the reason for visit.
  2. If the introduction field is blank, compose an introduction for the patient.
  3. If checked, click to clear the Do not Launch HPI check box.
  4. In the Reason for Visit data grid, click one of the reasons for visit entered previously to open the HPI template to add additional details.
  5. To add a Reasons for Visit do one of the following:
    • Select the appropriate reason for visit in the left pane to launch the associated HPI template.
    • Select the down arrow next the Other field to select a reason for visit from a list of specialty-specific reasons for visit.
    • Select Additional/Manage from the left pane to select a Reason For Visit from the subsequent window.
  6. In the HPI template, enter the appropriate data.
  7. Enter any comments.
  8. Click Save & Close.
  9. The entered information appears in a concatenated paragraph format under the History of Present Illness column in the Reason for Visit panel.
