The Reason for Visit panel is where the reason for visit, as described by the patient, is documented, along with an initial History of Present Illness (HPI). The provider should review reasons for visit, update if necessary, and complete HPI information. Reason for Visit If the Introduction field contains text, review the content and make any necessary changes. The introduction appears as the first sentence of the visit note, and it can be edited for the individual patient. A typical introduction might contain the patient's age, gender, and the reason for visit. If the introduction field is blank, compose an introduction for the patient. If checked, click to clear the Do not Launch HPI check box. In the Reason for Visit data grid, click one of the reasons for visit entered previously to open the HPI template to add additional details. To add a Reasons for Visit do one of the following: Select the appropriate reason for visit in the left pane to launch the associated HPI template. SelecDocument the Reason for Visit

The patient who presents for an Initial Prenatal visit is there because of pregnancy. It is not required to document the same in the Reason for Visit panel. The Reason for Visit panel on the Intake template is commonly used for pregnant patients who are visiting for a problem outside of the global pregnancy billing. Information entered in this section is not printed on the Prenatal Crystal report. If this section is used, either the Intake Note or the OB Master document is generated. The Reason for Visit panel on the Intake template displays the common reasons for a patient visit. You can select a reason for visit directly from the top list of common reasons for visit on the left pane. The Additional / Manage link opens the Reason For Visit template to access all reasons for visit. You can select any reason from the list of all reasons for visit or manually enter a reason for visit in one of the fields. To select a reason for visit, if desired: Open NextGen® Enterprise EHR. You can use the Intake template to document the patient information. You can document the following details of the patient in the various sections of the Intake template: General information of the patient Reason for the patient's to visit Patient's history Patient's Assessments Patient Medications Patient's allergies Vital Signs Patient's Orders Review of systems Patient Education Start a Dermatology Encounter Patient's General Information You can document a patient's general information on the Intake template. Add a Reason for Visit You can document the History of Present Illness (HPI), problems, as the reason for a patient visit. Patient's History Summary Patient's Assessment You can view patient's assessments. Patient Medication You can document and manage a patient's medications. Adding and Reviewing Allergies You can add and review the allergy details associated with a patient in the Allergies panel. You can document a patient's reasons for visit. On the Reason for Visit panel, you can document the common reasons for a patient visit. You can view the configurable specialty-specific reasons for visit list on the left pane. When you select a reason for visit that is already selected in the current encounter, a message appears highlighting the duplication. You can retain or remove the duplicate reason for visit. You can select up to 14 History of Present Illness (HPIs) simultaneously. Additionally, you can select the Do not launch HPI check box and any HPI to open the corresponding HPI template. Note: You can configure submenu items for the reason for visit on the Framework Content system template for the specialty. Based on the configuration, on the applicable template, the submenu items appear in the Reason for Visit panel on the left pane under the respective HPI. You can document a patient's reasons for visit. In the Reason for Visit panel, you can document the History of Present Illness (HPI), problems, and diagnosis history as the reason for a patient visit. When you select a reason for visit that is already selected in the current encounter, an alert appears indicating that the reason for visit is already selected. You can either enter the duplicate reason for visit, or remove the duplicate. For a reason for visit that has separate listings for different specialties, such as abrasion, if you select two HPIs from different specialties for the same reason, an alert appears. You can select up to fourteen HPIs simultaneously. Additionally, you can select the Do not launch HPI check box and can select any HPI to open the corresponding HPI template. You can add HPIs as the reasons for a patient's visit. Open NextGen® Enterprise EHR. On the Intake template, in the Reason for Visit panel, select Do not launch HPI. Note: You can configure the Do not launch HPI check box on the Practice Configuration template. To select the appropriate complaint, do one of the following: Select a link from the left pane in the Reason for Visit panel. Select Additional / Manage to choose maximum six reasons from the resulting template. Note: If the selected reason is a duplicate, a message appears highlighting the duplication. You can retain or remove the duplicate reason for visit. You can do one of the following: To save the duplicate and continue with the existing workflow, select Yes. To avoid the selection and select a different complaint, select No. Enter the first letter or first few le
