Add Reasons for Visits

You can add HPIs as the reasons for a patient's visit. Open NextGen® Enterprise EHR. On the Intake template, in the Reason for Visit panel, select Do not launch HPI. Note: You can configure the Do not launch HPI check box on the Practice Configuration template. For more information, go to NextGen Healthcare Success Community, and download the Practice Configuration Template User Guide forNextGen®Adaptive Content Engine. To select the appropriate complaint, do one of the following: Select a link from the left pane in the Reason for Visit panel. Select Additional / Manage to choose maximum six reasons from the resulting template. Note: If the selected reason is a duplicate, a message appears highlighting the duplication. You can retain or remove the duplicate reason for visit. You can do one of the following: To save the duplicate and continue with the existing workflow, select Yes. Document Reasons for a Patient's Visit

You can document up to six reasons for a patient's visit during intake. OpenNextGen® Enterprise EHR. Open theIntaketemplate, and then expand theReason for Visitpanel. To add reasons for the patient's visit without opening another template, select Do not launch HPI, and then do one of the following: Select the reason for visit from the left navigation bar. To add a customized reason, enter the reason in Other, and then select Add. Select Additional / Manage, select appropriate reasons from the Reason for Visit window, and then select Save and Close. Note: You can view all the available history of present illnesses (HPIs) by selecting the All hpis checkbox in the Reason for Visit window. To add reasons for the patient's visit with additional details by using another template, clear the Do not launch HPI checkbox, select the necessary reasons, and then select Save & Close . Set Up HPIs to Appear in the Reason for Visit

You can set up the History of Present Illness (HPI) to appear in the reason for visit. A practice can configure the Reason for Visit panel using the Framework Content system template. For more information on the reason for visit configuration, go to NextGen Healthcare Success Community, and download the latest Framework Content System Template forNextGen®Adaptive Content Engine. Open NextGen® Enterprise EHR. From the File menu, select System \ Practice Template. The Select Template window opens. In the Show section, select All. To open the Framework Content template, double-click Framework Content. Select Specialty and select a specialty from the list. Expand the Reason for Visit panel. In Filter, select the all option. Select the reason for visit linked to the appropriate History of Present Illness (HPI), which you want to add to the all list in the Reason for Visit list. Note: The reasons for visit are linked to an HPI template on the Manage Custom Links practice template. Select OK.

Patient's Reason for Visit

You can document a patient's reasons for visit. In the Reason for Visit panel, you can document the History of Present Illness (HPI), problems, and diagnosis history as the reason for a patient visit. When you select a reason for visit that is already selected in the current encounter, an alert appears indicating that the reason for visit is already selected. You can either enter the duplicate reason for visit, or remove the duplicate. For a reason for visit that has separate listings for different specialties, such as abrasion, if you select two HPIs from different specialties for the same reason, an alert appears. You can select up to fourteen HPIs simultaneously. Additionally, you can select the Do not launch HPI check box and can select any HPI to open the corresponding HPI template. Note: You can configure sub menu items for reason for visit on the Framework Content system template for the specialty. Add a Reason for Visit

You can add up to six HPIs as the reason for a patient visit for an encounter. Open the Intake template. Expand the Reason for Visit panel. To do a full skin exam, in the Complete skin exam desired? question, click the Yes option. To add a reason for visit without additional details select the Do not launch HPI check box. To add a reason for visit with additional details on the HPI template, clear the Do not launch HPI check box. In the left pane, click the needed entries. The selected reasons for visit appear in the Reason for Visit column. If the reason for visit is not listed in the panel, do the following: Click the Other list and select the reason for visit from the resulting list. You can also type a reason in the Other field and click Add. To document any details, click the Intake Comments link. The Intake Comments window opens. Enter comments and click Save & Close. Document the Patient's Reason for Visit as a Provider

The reason for visit added by the intake staff on the Intake template appears on the SOAP template. The Introduction field is an editable field that displays a brief summary of the patient's reason for visit. Open NextGen® Enterprise EHR. On the SOAP template, in the Reason for Visit panel, do one of the following: Select one of the reasons for visit entered previously. Select a complaint from the reasons for visits in the left pane. The associated HPI template opens. Note: If you select reason(s) for visit using the Additional / Manage link, then the reasons appear in the Chief Complaint column in the data grid. You must select the reasons listed in the Chief Complaint column in the data grid to open the associated HPI template. Reason for Visit panel On the associated HPI template, enter the appropriate data. Enter any comments. Select Save & Close. The entered details appear in the History of Present Illness column in the Reason for Visit panel. Parent topic: SOAP Template
