You can document up to six reasons for a patient's visit during intake.
- OpenNextGen® Enterprise EHR.
- Open theIntaketemplate, and then expand theReason for Visitpanel.
- To add reasons for the patient's visit without opening another template, select Do not launch HPI, and then do one of the following:
- Select the reason for visit from the left navigation bar.
- To add a customized reason, enter the reason in Other, and then select Add.
- Select Additional / Manage, select appropriate reasons from the Reason for Visit window, and then select Save and Close.
Note: You can view all the available history of present illnesses (HPIs) by selecting the All hpis checkbox in the Reason for Visit window.
- To add reasons for the patient's visit with additional details by using another template, clear the Do not launch HPI checkbox, select the necessary reasons, and then select Save & Close .
- To copy reasons from a previous encounter to the current encounter, select HPI History.
If you have copied information from a previous encounter, an asterisk (*) appears in the
Copied column of the
Reason for Visit grid.
Note: You can configure the functionality to copy reasons for visit from previous encounters in the
Copy Previous Visit Information section of the
Practice Configuration template. Without the configuration, the
Historical Information - HPI
window opens in read-only mode when you select HPI History.

- To update a reason as a follow-up, select Additional / Manage, and then select the Follow up checkbox for the respective reason.
- To document the diagnostic studies, select Diagnostics.
Note: Diagnostic studies added to the HPI grid from the Diagnostic History Entry template appear in the Reason for Visit grid as Studies Reviewed.
- To document the chronic conditions, select Chronic Conditions.
- To view the HPI details for all chief complaints, select View All.
- To enter intake comments, select Intake Comments.
