Document Contraception Information

  1. Open the Intake template.
  2. Expand the Reason for Visit panel.
  3. Select Contraception as the reason for the current visit.
    The Contraception – HPI template opens, and contraception appears in the Concern field.

  4. To indicate a follow-up visit for contraception, select the Follow Up check box.
    The Concern field is updated to Follow-up of contraception.

  5. Document the following information:
    • Current and desired birth control method
    • Reason for no birth control (at Intake)
    • Desired birth control method
    • Pros and cons of the current method
    • Secondary conditions the patient is hoping to improve with birth control
    • Previous methods, the age when the methods were used, and their responses
    • Methods the patient does not consider and why
    • If the patient is currently pregnant
    • If the patient would like to get pregnant in the next year and the time frame
    • If the patient has had unprotected intercourse within last five days or since LMP
  6. To include all confidential information in the social history document with patient permission, select the corresponding check box.

  7. Document the intimate partner violence screening, menses details, and possible contraindications.
  8. To document the social history and suicidal or homicidal risk of the patient, select Tobacco/Vaping/Alcohol and Suicidal/Homicidal screening respectively.
  9. For review of systems, indicate presence or absence of the symptoms.
    You can use check boxes to indicate any associated symptoms, pertinent negatives, or others negatives.
  10. To include additional information, manually enter your comments or use My Phrases.
  11. Select Save & Close.
    The saved record appears in the Reason for Visit panel.
