You can document the following details of the patient in the various panels of the Intake template: General details of the patient Reason for the patient's to visit Patient's history Vital signs Patient's allergies Medications details Review of systems Orders You can select the Record contains substance use disorder information check box to include the disclosure for Substance Abuse and Mental Health Services Administration (SAMHSA) 42 CFR Part 2 provisions in the generated documents and reports. The intake staff uses the Intake template to document a patient's chief complaint, vital signs, medications, allergies, standing orders and various other findings. Documenting General Information of Patients You can document a patient's general information on the Intake template. Document Reasons for a Patient's Visit You can document up to six reasons for a patient's visit during intake. The information documented by the intake staff is used by the provider for reference and to proceed with the encounter. Open the Home Page Template You can obtain the patient summary on the Home Page template. OB Intake Staff Workflow

The intake staff workflow for an Initial Prenatal visit involves documenting the patient encounter on the Intake, Gestational Age, Histories, and Education templates. The information documented by the intake staff is reviewed by the provider. When you access the Intake template, select Obstetrics and OB Prenatal Initial from the Specialty and Visit Type fields to display the Initial Prenatal template set. OB Intake tab The Intake template includes various links to add more information about the patient.
