Document a Treatment Plan

You can document treatment plan for a patient. Before you begin You must enter a diagnosis for the patient before documenting a treatment plan. Open the ASC - Physician Procedure template. In the Assessment / Plan panel, click the My Plan link. The My Plan template opens. If required, select the assessment in the Today's Assessments field. The My Plan template, which is part of a group of templates, uses the last assessment or diagnosis selected on any of the templates in the group as the working assessment. The system defaults to the current working assessment. You can select another assessment, which becomes the new working assessment that carries forward to the other templates in the group. The selected assessment appears in the Selected Dx field. The system displays the order set for the associated diagnosis category. My Plan vs. My Custom Plan to Create a Treatment Plan

If configured, the My Plan tab houses an enriched plan template called My Custom Plan, which can be used to document a treatment plan that is personalized to suit your practice's specific needs. The custom plan functionality is configured as a grid for order sets, replacing the need to manually select a check box for each order to be placed. The individualized fields available on the original My Plan require manual selection of every order needed for a particular diagnosis. The purpose for the My Custom Plan template is to reduce the number of clicks required for ordering while maintaining the process for managing order sets and provider plans. The My Custom Plan template still takes a personalized approach to diagnosis-based order sets. The custom plan template simply uses a more streamlined user interface. The treatment plan for the patient can be documented on the My Plan or My Custom Plan template. The My Plan template appears within NextGen® Adaptive Content Engine by default and can be replaced with the My Custom Plan template based on the configuration in the Practice Configuration practice template. The treatment plan for the patient can be documented on the My Plan (PT) template that enables providers to place multiple orders from a single template. The My Plan (PT) template is based on order sets for diagnosis categories that can be modified and managed by clients using practice templates. The My Plan (PT) template appears within NextGen® Adaptive Content Engine by default and can be easily replaced with the My Custom Plan template after a simple configuration in the Practice Configuration practice template. The My Custom Plan template can be personalized to suit your practice's specific needs and is less click-intensive. Diabetic Treatment Plans

The My Plan template contains a preconfigured order set for diabetes. Practices can customize the order sets at the practice or provider level. This clinical decision support prompts the provider to consider certain tests and instructions for diabetic patients. Items in the Instruction section are user defined and disease specific. They give credit toward Quality Reporting education and instruction. Note: The Additional orderssection can be configured to provide additional instructions or lab orders. Create a New Order Set

The My Plan templates display all order sets that are available in the system, including those that are shipped with NextGen® Adaptive Content Engine. The order sets can be based on a diagnosis group, provider, or specialty (department). To manage clinical references, configure categories for order types or the master picklist, refer to the indicated link. On the Assessment Plan Customization template, choose a Display option to filter the grid under the Order Sets (My Plan/Custom Plan) section by the Current provider or All providers. In the grid under the Order Sets (My Plan/Custom Plan) section, right-click and select Add New. The Provider's Custom Plan template opens. Depending on how you want to set up the order set, click the Diagnosis Group, Provider, and/or Dept fields to select a value from the resulting list.
