Document Adverse Childhood Experiences Screening

You can document Adverse Childhood Experiences (ACE) of patients aged between 0 to 19 years using the Adverse Childhood Experiences (ACE-Q) template. Open NextGen® Enterprise EHR. For a patient encounter, open a template.For example, the Intake template. In the header link section, select Screening Tools.The Screening Tools template opens. Select Adverse Childhood Experiences (ACE-Q).The Adverse Childhood Experiences (ACE-Q) template opens. The encounter date is the default date in the Performed Date field. You can manage patient details from the template header links. Template header links are available on each BH medical record template. Document the Ohio Scales for Youth Template

Some state behavioral and mental health agencies may require Community Behavioral Health (CBH) practices to complete a specific screening tool. Hence, NextGen Behavioral Health Suite provides Ohio Youth Problem, Functioning, and Satisfaction Scales, otherwise known as Ohio Scales for Youth agency worker form template . The Ohio Scales for Youth are brief measures of outcome for youth receiving mental health services. The scales include a 20 item Problem Severity Scale. You can document details provided by the patient in all the screening and assessment tools configured by the practice during a patient encounter. Screening tools that appear on the Screening Tools template for a selected specialty in an encounter, vary based on the configuration. When you do not select a specialty for an encounter, all the screening tools automatically appear on the Screening Tools template. You can select the Current Specialty or All option to filter the screening tools on the Screening Tools template. The following indicators appear adjacent to the screening tools and indicate whether the screening tool opens as a template or a website. An asterisk * indicates that the screening tool is used during the current encounter. The letter T indicates that the screening tool opens as a template. The letter W indicates that the screening tool opens a website. Note: You can hide a screening tool that is configured to open as a website. For more information on screening tools l

Document Suicide Risk Evaluation Using the Columbia Suicide Severity Rating Scale

The Columbia Suicide Severity Risk Scale (C-SSRS) screening tool is part of the NextGen Behavioral Health Suite workflow. Developed by researchers at Columbia University, University of Pennsylvania, University of Pittsburgh, and New York University, the C-SSRS is designed to assess suicidal ideation and behavior. It rates the severity of suicidal ideation on a scale from "wish to be dead" to "active suicidal ideation with specific plan and intent and behaviors." Questions are phrased for use in an interview format, but the C-SSRS can also be completed as a self-report measure if necessary. The scale identifies behaviors that may indicate an individual's intent to complete suicide.
