You can use the Screening Tools template to document screenings for items that are not listed under the Screening Tools Available list. On the Screening Tools template, select a screening tool from the Screening Tools Available section. Enter the applicable information: Score Severity or interpretation Comments Major depressive disorder pre-treatment classification Plan To indicate an associated scanned document, select the See scanned document checkbox. Select Add. The documented data appears in the Screening Tools Performed grid. To print a list of screening tools during this encounter, select the notepad located next to the Screening Tools link in the template header. To access the AIMS Exam (BH) template, select Select Template. The Select Template window opens. Select All to display all available templates. Select AIMS Exam (BH). The AIMS Exam (BH) template opens. On the AIMS Exam (BH) template, select the correct number on the scale for each section. To mark all questions as normal (value of either 0 or No), select the Default All Normal (0 or No) checkbox. You can manually change the answer, if applicable. You can document the functional status assessment for chronic conditions. Open NextGen® Enterprise EHR. Select a patient encounter, and open a template. For example, Intake. Select Navigation, and then select the Document Library link. The Documents and Letters template opens. Select the Veterans Rand 12 Item Health Survey (VR-12) link. Print and give the Veterans Rand 12 Item Health Survey (VR-12) questionnaire to the patient for completion. Open the navigation bar, and then select Screening Tools. You can document information in a screening tool that opens as a template. Open the NTS Screening Tool template. In the Screening Tools Available list, select a screening tool that has a T indicator in the first column. Note: An asterisk (*) denotes a screening tool that has already been used in the current encounter. The corresponding screening tool template opens. Complete entries for the patient on the screening tool. If the template does not keep a running score, select the Calculate button to calculate the score. Save and close the screening tool. Note: The button to save and close the template may be labeled Save & Close, OK, or Accept. The screening tool name and results are saved in the Screening Tools Performed grid on the NTS Screening Tool template. Complete any other screening. Select the Risk Factors link. The Risk Factors Config template opens. Risk Factors Config template Select the No risk indicators checkbox if the patient has no risk conditions, and then select Save & Close. For patients who have one or more of the risk conditions, do the following: In the Tobacco section, select Tobacco Usage to open the Social History - Tobacco template. Note: If the tobacco information is updated on the Screening Summary template or on the Risk Factors Config template, the application fills values in the Social History - Tobacco template.
