Risk Factors

You can document the risk factors for tobacco use (TOB), hypertension (HTN), diabetes (DM), and coronary artery disease (CAD) on the Risk Factors Config template. These are not the conditions that the patient is considered to be at high risk for. If you want to indicate that the patient is at high risk for developing a certain condition, you can document such information on the Care Guidelines template. Document the Risk Factors

You can document the risk factors for patients. Open NextGen® Enterprise EHR. On a patient encounter, open any of the standard visit templates. (for example, the Intake template. On the template header, select Config. The Risk Factors Config template opens. If the patient has no risk factors, select No risk indicators, and select Save & Close. If the patient has a history of tobacco use, do the following: In the Tobacco section, select Tobacco Usage.The Social History - Tobacco template opens. Note: If the tobacco information is updated on the Screening Summary template or on the Risk Factors Config template, the application automatically fills values on the Social History - Tobacco template. Enter data on the Social History - Tobacco template, and select OK. Select the Tobacco Cessation Discussed link.The Tobacco Cessation Discussed template opens. Document Risk Factor Indicators

Click the Configure icon on the top corner of the template.The Oph Risk Factors Config template opens. oph risk factors config If the patient has no risk conditions, select the No risk indicators check box and then click Save & Close. Select Yes, No, or Unknown for the following risk factors. Tobacco Cessation

You can use the Tobacco Cessation template to document the patient's tobacco use and the methods used for tobacco cessation. You can open the Tobacco Cessation template by using the Tobacco Cessation link. The Tobacco Cessation template enables you to document the tobacco history details, complete the medication screening questions, select the methods used for tobacco cessation, add assessments, and select and schedule the recommended treatments. If the patient has a medical condition due to tobacco use, then you can use the Tobacco Cessation template as a stand-alone visit for tobacco cessation. The Submit to Superbill button on the template enables you to submit the charges for the template. Note: For counseling sessions of less than 3 minutes, you can utilize the Risk Factors Config template to document the tobacco cessation discussed with the patient instead of the Tobacco Cessation template. The Tobacco Cessation sub-navigational link opens the Tobacco Cessation template. You can use the Tobacco Cessation template to document tobacco history details, complete the medication screening questions, select the methods used for tobacco cessation, add assessments, and select and schedule the recommended treatments. The template enables in-depth evaluation and comprehensive documentation of the patient's tobacco use and the methods used for tobacco cessation. The Submit to Superbill button on the template enables you to submit the charges for the template. Note: The Tobacco Cessation template can be used as a stand-alone visit for tobacco cessation for the patients who have medical conditions due to tobacco use. For counseling sessions of less than 3 minutes, you can utilize the Risk Factors Config template to document the tobacco cessation discussed with the patient. Record the Patient's Derm Risk Factors

Open NextGen® Enterprise EHR and select a Established patient or New patient as appropriate. Create a patient encounter and open the Intake template. Select a Specialty of Dermatology and a Visit Type of Office Visit. The Derm Risk Factors Config link is available on the patient header of every dermatology template Click the Config icon. The Derm Risk Factors Config template opens. If the patient does not have any risk factor, select the No risk indicators check box. In the Tobacco section, document the smoking status of the patient as follows: Click Tobacco Usage to document the tobacco consumption details such as tobacco type and usage per day on the Social - History Tobacco template. Click Tobacco cessation discussed to document smoking cessation efforts and passive smoke exposure on the Tobacco Cessation Discussed template. Select Yes, No, or Unknown for other risk factors. The default setting is Unknown. The risk factors for Melanoma (MM), Diabetes (DM), Basal cell carcinoma (BCC)
