Document Tobacco Cessation Efforts
On the
Social History - Tobacco
template, expand the
Efforts to Quit Tobacco
panel.
Select from the following options for
Have you ever tried to quit using tobacco
:
No/Never
Yes
Unknown
Note:
Selecting either
No/never
or
Unknown
means that there is no other documentation required on this panel.
Enter
Tobacco type.
To document that the client no longer smokes, enter the
Month
,
Day
, and
Year
(must be four digits) next to
Quit
.
Click the
Longest tobacco free
field to enter the number of days, weeks, months, or years the client quit using tobacco.
Enter the
Cessation method
.
Type the reason in the
Relapse reason
field.
Click
Add
to enter the information into the grid.
To document your conversation about smoking cessation methods, click the
Tobacco cessation discussed
checkbox.
The
Tobacco Cessation Discussed
template launches.
Choose one of the following options:
Single select/Close
- to make a single selection and close the template automatically
Multiple selection
- to select multiple cessation methods and then click
Save & Close
.
The selected
Discussion
type and
Completion Date
move to the
History of Tobacco Cessation Discussions
grid.

Enter Vaping Data
On the Social History - Tobacco template, expand the Vaping Use panel. Note: The fields in this panel are read-only until a Yes response is documented for Screened for vaping. Select the Yes option for Screened for vaping and document the client's response for Status and Duration. Click any of the following fields and select an appropriate option from the corresponding list: Device type Frequency Current strength - the current nicotine percentage Previous strength and Duration Select one of the options for Vaping reason. Enter Tobacco Usage
The client's current smoking status must be recorded to meet Quality Reporting requirements. The Social History - Tobacco template in the Behavioral Health template set only includes the Tobacco option as the other social history data is documented in different sections of the template set. This ensures that users are documenting the required data and prevents duplicate information from printing in the documents. In the Substance Use/Addictive Behavior History panel, click the Tobacco Usage link. Select one of the following options for Have you ever used tobacco?: No/never Yes Unknown If you select No/never or Unknown, click Save & Close to return to your previous workflow. Select Yes or No for Have you used tobacco in the last 30 days? Select Yes or No for Have you used smokeless tobacco products in the last 30 days? Note: For the current encounter, if you select No\Never for Have you ever used tobacco?, the system selects No for both questions. If you have selected No/Never in the pr

Document Tobacco Cessation Efforts
You can document a patient's tobacco cessation efforts in the Efforts to Quit Tobacco panel on the Social History – Tobacco template. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. On the Social History – Tobacco template, expand the Efforts to Quit Tobacco panel. Select the Have you ever tried to quit using tobacco? option. If you select No/never, document details of the discussions on tobacco cessation by selecting the Tobacco cessation discussed link. Social History – Tobacco To add the details of the patient's efforts to quit tobacco, do the following: Select the tobacco type, and then enter the date the patient attempted to quit tobacco. Document Methods of Tobacco Cessation
You can document in-depth evaluation and comprehensive information about a patient's tobacco use and methods for tobacco cessation. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. The Social History – Tobacco template opens. Expand Efforts To Quit Tobacco. From the Tobacco Cessation Information section, select the Tobacco cessation discussed link. You can also access this link from other templates. The Tobacco Cessation Discussed window opens. Add Social History
Before you begin You can configure the show confidential history, panel version of adult and pediatric social history templates from the Social History section of the Practice Configuration template. For more information about these settings, go to NextGen Healthcare Success Community and download the Practice Configuration Template User Guide for NextGen® Adaptive Content Engine You can add social history information in the History Summary section. The Social section uses age-specific templates based on the patient age and the Pediatric Age configuration on the Practice Configuration practice template. Open NextGen® Enterprise EHR. Open the Intake template. In the History Summary section, select Social. Select Select Social Hx to Add/Edit and select the relevant option from the list.Based on the patient age, the relevant social history template opens. Document the reported history information. Manage Tobacco Cessation Discussions
You can replace or remove tobacco cessation discussion items. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. Expand Efforts To Quit Tobacco. In the Tobacco Cessation Information section, select the Tobacco cessation discussed link. The Tobacco Cessation Discussed template opens. To remove a discussion item, do the following: In the History of Tobacco Cessation Discussions section, select the discussion item that you want to remove. select Remove. To replace a discussion item, do the following: In the History of Tobacco Cessation Discussions section, select the discussion item that you want to replace.
