View Historical Tobacco Usage
On the Social History- Tobacco template, expand the Historical Use panel. If the client has encounters entered on a NextGen® Adaptive Content Engine release prior to version 7.9.1, click the Click here to see tobacco history prior to 7.9.1 link. The Tobacco Use History template opens. Click Save & Close to return to the Social History - Tobacco template to review the client's tobacco history that was documented in versions later than 7.9.1. Document Tobacco Cessation Efforts
On the Social History - Tobacco template, expand the Efforts to Quit Tobacco panel. Select from the following options for Have you ever tried to quit using tobacco: No/Never Yes Unknown Note: Selecting either No/never or Unknown means that there is no other documentation required on this panel. Enter Tobacco type. To document that the client no longer smokes, enter the Month, Day, and Year (must be four digits) next to Quit. Click the Longest tobacco free field to enter the number of days, weeks, months, or years the client quit using tobacco. Enter the Cessation method. Type the reason in the Relapse reason field. Click Add to enter the information into the grid. To document your conversation about smoking cessation methods, click the Tobacco cessation discussed checkbox.The Tobacco Cessation Discussed template launches. Enter Tobacco Usage
The client's current smoking status must be recorded to meet Quality Reporting requirements. The Social History - Tobacco template in the Behavioral Health template set only includes the Tobacco option as the other social history data is documented in different sections of the template set. This ensures that users are documenting the required data and prevents duplicate information from printing in the documents. In the Substance Use/Addictive Behavior History panel, click the Tobacco Usage link. Select one of the following options for Have you ever used tobacco?: No/never Yes Unknown If you select No/never or Unknown, click Save & Close to return to your previous workflow. Select Yes or No for Have you used tobacco in the last 30 days? Select Yes or No for Have you used smokeless tobacco products in the last 30 days? Document Vaping Details of a Patient
The Tobacco/Vaping Use panel in the Social History - Tobacco template enables you to screen and document the vaping details of a patient. The documented vaping details are carried forward to future encounters. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. The Social History – Tobacco template opens. Expand the Vaping Use panel. In the Screened for vaping field, select Yes. Select a Status for vaping. If the patient is a current user, select a number in the Age started field. If the patient is not a current user, select a number in the Age started and Age stopped fields. Document the Reason for Not Screening a Patient for Vaping
You can document the reason for not screening a patient for vaping in using the Social History – Tobacco template. Open the Histories template. In the Social panel, selects Add.The Social History – Tobacco template opens. Expand Vaping Use. Note: The fields in this panel are unavailable until you select the Yes option for Screened for vaping. For Screened for vaping, select No. In Vaping comments, enter the reason for not screening the patient for vaping.
