You can document the risk factors for patients.
 
  - Open NextGen® Enterprise EHR.
 
  - On a patient encounter, open any of the standard visit templates. (for example, the Intake template.
 
  - On the template header, select Config
. 
   
    The 
Risk Factors Config template opens. 
    
    
  - If the patient has no risk factors, select No risk indicators, and select Save & Close.
 
  - If the patient has a history of tobacco use, do the following:
   
    - In the Tobacco section, select Tobacco Usage.
     
      The 
Social History - Tobacco template opens. 
      
Note: If the tobacco information is updated on the Screening Summary template or on the Risk Factors Config template, the application automatically fills values on the Social History - Tobacco template.
      
 
      
    - Enter data on the Social History - Tobacco template, and select OK.
 
    - Select the Tobacco Cessation Discussed link.
     
      The Tobacco Cessation Discussed template opens.
     
 
    - Enter information about the tobacco cessation discussion, and select Save and Close.
 
    - Select 
 for quality reporting. 
    - Save and close the template.
     
      The tobacco use data appears in the respective fields and data grids.
     
 
   
 
  - To indicate the risk for hypertension, diabetes, and coronary artery disease (CAD), select Yes, No, or Unknown.
 
  - Select Save & Close.
