Document the Risk Factors

You can document the risk factors for patients.

1. Open NextGen® Enterprise EHR.
2. On a patient encounter, open any of the standard visit templates. (for example, the Intake template.
3. On the template header, select Config.
   The Risk Factors Config template opens.

   

4. If the patient has no risk factors, select No risk indicators, and select Save & Close.
5. If the patient has a history of tobacco use, do the following:
   1. In the Tobacco section, select Tobacco Usage.
      The Social History - Tobacco template opens.

      Note: If the tobacco information is updated on the Screening Summary template or on the Risk Factors Config template, the application automatically fills values on the Social History - Tobacco template.
   2. Enter data on the Social History - Tobacco template, and select OK.
   3. Select the Tobacco Cessation Discussed link.
      The Tobacco Cessation Discussed template opens.
   4. Enter information about the tobacco cessation discussion, and select Save and Close.
   5. Select for quality reporting.
   6. Save and close the template.
      The tobacco use data appears in the respective fields and data grids.
6. To indicate the risk for hypertension, diabetes, and coronary artery disease (CAD), select Yes, No, or Unknown.
7. Select Save & Close.
