Document Post-traumatic Assessment Details using the PCL-5 Template

Open NextGen® Enterprise EHR. For a patient encounter, open a template. Note: Your practice can also configure the template to be accessible from thePacket Navigation (BH)template. For more information, go toNextGen Healthcare Success Community, and download theConfiguration Guide forNextGen® Behavioral Health Suite. Select the Screening Tools link. The Screening Tools template opens. Select PSTD Checklist for DSM-5 (PCL-5). Select the assessment date. Select a severity score for each question. The total score is calculated based on the selected scores for all of the questions. Select Save & Close. The information appears in the Screening Tools Performed grid on the Screening Tool window. Document Patient Screening

You can document patient screening in the Screening panel of the SBIRT Screening template. Open NextGen® Enterprise EHR. In the navigation bar, select Screening Tools. The Screening Tools template opens. In the Screening Tools Available section, select Screening, Brief Intervention, and Referral to Treatment (SBIRT). The SBIRT Screening template opens. SBIRT Screening Template Expand the Screening panel.Screening Enter the details related to screening instrument, score, severity or interpretation, comments, and Major Depressive Disorder (MDD).Performed Date is filled automatically. Document Patient's Mental Health Details Using the Brief Assessment Template

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. From the list, select Brief Assessment (BH), and then select OK. Select the program, specialty, and visit type for the patient. Select whether you have discussed confidentiality and obtained patient's consent for the care. To document the patient's past medical history, select the History Summary link. In the Presenting Concerns panel, do one of the following actions in the Written HPI narrative field: Enter the patient's symptoms and presenting issues that prompted a visit to the office. Use the My Phrases link to select the details for the visit. In the Substance Use/Addictive Behavior panel, select the patient's addiction details to substances. Document the Patient's Referrals for Psychiatric Rehabilitation

You can document referrals, treatment recommendations, assessed needs, and discharge planning for psychiatric rehabilitation patients. You can document the directed care plan and services that are provided as part of the psychiatric rehabilitation services. Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Rehab Referral (BH), and then select OK. Note: You can open the Rehab Referral (BH) template from your Packet Navigation,or from the Templates module. Document SBIRT Screening

You can document SBIRT screening information to analyze a patient's need for intervention or for referral to treatment. Open NextGen® Enterprise EHR. In the navigation bar, select Screening Tools. The Screening Tools template opens. In the Screening Tools Available section, select Screening, Brief Intervention, and Referral to Treatment (SBIRT). The SBIRT Screening template opens. SBIRT Screening Template Select a performed date. Enter Assessment and Diagnosis Details

Expand the Assessment/Diagnosis panel. If enabled, click the DSM IV link in the upper, right corner to launch the BH – DSM IV template. Note: The display of the link is set up on the Behavioral Health section of the Practice Configuration system template. In the Behavioral Health Diagnoses section, select any of the available links. In the Diagnostic description required field, click to launch the diagnosis search window. Selecting a description value automatically populates the ICD Code field or vice versa. Add Status, Severity, Differential DX, and Impression. The out-of-the-box Severity picklist does not include Extreme or Persistent. These values can be added to the list in File Maintenance. Enter any required Specifiers by doing one of the following actions: Click the field to type a value. Click the My Phrases link to create a new sentence or part of a sentence.
