Document Psychosocial Stressors and GAF Score

From the Assessment/Diagnosis panel, select the Psychosocial Stressors link. The Tis Bh Psycho Stressors window opens. Select the severity. For Problems related to questions, select either No or Yes. If Yes, provide additional information in the text field. Optionally, use Comments to enter additional comments. In Current GAF, enter the score. To view the GAF scale, select the GAF Scale link. The date fills in with today’s date. If this is the first recorded GAF score, the Highest GAF in past year and Date fields fill in with according to the Current GAF information. To edit the date, select the date and then select a date from the calendar. Select Save & Close. To document a referral, select the Referral link. The Referral link opens the Referral Management template. The Referral Management template can be integrated with NextGen® Share to complete electronic outgoing referrals. Document Psychiatric Screening Details in the BPRS Template

You can quickly gather information about the possible presence and severity of various psychiatric symptoms using the Brief Psychiatric Rating Scale (BPRS) template. Open NextGen® Enterprise EHR. For a patient encounter, open a template. In the header link section, select Screening Tools. The Screening Tools template opens. Select BH Brief Psychiatric Rating Scale (BPRS). The BH Brief Psychiatric Rating Scale (BPRS) template opens. The encounter date is the default date in the Performed Date field. BH Brief Psychiatric Rating Scale (BPRS) Optional: Select an alternate date in the Performed Date field. Rate all the listed problems based on the condition of the patient. In the Interpretation of Results section, select Calculate Score. A two-digit score appears in Total Score. Based on the combined total score, a default interpretation appears in Interpretation. The WHO Disability Assessment Schedule 2.0 (WHODAS 2.0) link can be found in the Assessment/Diagnosis panel of all billable BH templates or from the Screening Tools template located from the template header link. From the BH - WHODAS template, complete questions S1 – S12. Note: Raw score or Score percentage does not display unless all questions are completed. For questions H1-H3, use the number pad to enter the number of days. The Raw score and Score percentage calculates and displays in the corresponding fields. Select Save & Close. The Raw score and Score percentage display on the Assessment/Diagnosis panel of the encounter. A record of the screening also displays in the Screening Tool grid on the Screening Tool window. Behavioral Health Assessment and Diagnosis

You can link a diagnosis to the current encounter and enter assessment details from the Assessment/Diagnosis panel. Practices that use institutional billing must provide an admitting diagnosis. You can enter the diagnosis in the Admitting Diagnosis field. If your practice does not use institutional billing, this field is not required and can be left blank. You can use the following links on the panel to search for a diagnosis code: Active Diagnosis allows you to use a diagnosis from a previous encounter for the client. Diagnosis Code Lookup allows you to add a diagnosis to the client’s medical chart. This should only be completed by staff members authorized to assess and diagnosis a client.
