Document WHODAS Response
The WHO Disability Assessment Schedule 2.0 (WHODAS 2.0) link can be found in the Assessment/Diagnosis panel of all billable BH templates or from the Screening Tools template located from the template header link. From the BH - WHODAS template, complete questions S1 – S12. Note: Raw score or Score percentage does not display unless all questions are completed. For questions H1-H3, use the number pad to enter the number of days. The Raw score and Score percentage calculates and displays in the corresponding fields. Select Save & Close. The Raw score and Score percentage display on the Assessment/Diagnosis panel of the encounter. A record of the screening also displays in the Screening Tool grid on the Screening Tool window. WHO Disability Assessment Schedule 2.0 (WHODAS 2.0)
The BH - WHODAS template is available with the Behavioral Health template set. It is a generic assessment instrument for health and disability, and a tool to produce standardized disability levels and profiles documentation. This tool is provided by the World Health Organization Disability Assessment Schedule (WHODAS) 2.0. After you complete the assessment questions, the Raw Score and Score Index generate automatically. After you document the WHODAS 2.0, a record of the screening displays in the Screening Tool grid on the Screening Tools window. Note: A link to the BH - WHODAS template opens when the Enable/Show WHODAS data on Assessment/Diagnosis option is enabled on the BH Foundation Configuration template. Document Discharge Summary
Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Discharge Summary (BH), and then select OK. Select the program, specialty, and visit type. Select the patient's discharge date. Document whether you need to close the selected program for the patient. If yes, select the status of the patient treatment in the program. In the Assessment/Diagnosis panel, document the patient's active diagnosis and assessment details. Select the WHODAS 2.0 link to complete the questionnaire about the patient's challenges due to health conditions. In the Discharge Summary panel, document the treatment details, post-discharge support details, risk details, and the reason for the patient's discharge. Document General Screening Tool Information
You can use the Screening Tools template to document screenings for items that are not listed under the Screening Tools Available list. On the Screening Tools template, select a screening tool from the Screening Tools Available section. Enter the applicable information: Score Severity or interpretation Comments Major depressive disorder pre-treatment classification Plan To indicate an associated scanned document, select the See scanned document checkbox. Select Add. The documented data appears in the Screening Tools Performed grid. To print a list of screening tools during this encounter, select the notepad located next to the Screening Tools link in the template header. Screening Tools and Follow-Up Plans of Care
You can document the details of the screening tools and follow-up plan of care for a patient. You can document the details of an assessment, a template, or website screening tool, and calculate the associated score using the Screening Tools link. If an assessment is not available, you can select a tool in the Screening instrument section and document the assessment. The documented data appears in the Screening Tools Performed grid. To document the follow-up plan of care in the Functional Status panel, you can send a notification to the desired recipients using the Task button. Document Information with Website Screening Tools You can document information in the screening tools that open as websites using the Screening Tools template. Behavioral Health Assessment and Diagnosis
You can link a diagnosis to the current encounter and enter assessment details from the Assessment/Diagnosis panel. Practices that use institutional billing must provide an admitting diagnosis. You can enter the diagnosis in the Admitting Diagnosis field. If your practice does not use institutional billing, this field is not required and can be left blank. You can use the following links on the panel to search for a diagnosis code: Active Diagnosis allows you to use a diagnosis from a previous encounter for the client. Diagnosis Code Lookup allows you to add a diagnosis to the client’s medical chart. This should only be completed by staff members authorized to assess and diagnosis a client.
