Document the Ohio Scales for Youth Template

Some state behavioral and mental health agencies may require Community Behavioral Health (CBH) practices to complete a specific screening tool. Hence, NextGen Behavioral Health Suite provides Ohio Youth Problem, Functioning, and Satisfaction Scales, otherwise known as Ohio Scales for Youth agency worker form template . The Ohio Scales for Youth are brief measures of outcome for youth receiving mental health services. The scales include a 20 item Problem Severity Scale. The agency worker form also includes the Restrictiveness of Living Environment Scales (ROLES) and several indicators of outcome.
  1. Open NextGen® Enterprise EHR.
  2. For a patient encounter, open a template.
    For example, Intake.
  3. In the header link section, select Screening Tools.
    The Screening Tools template opens.
  4. Select Ohio Scales for Youth.
    The Ohio Scales for Youth template opens. The Performed Date defaults to the encounter date.

  5. Optional: To choose a different Performed Date, select in the field and select a date.
  6. Expand the Problem Severity panel, rate all the listed problems based on the experience of the patient and then select Calculate.
    The cumulative score of the rating is calculated and displayed in the Total Score section.
  7. Expand the Restrictiveness of Living Environments panel, document the living environment of the patient from past 90 days and then select Calculate.
    The score of the documented values is calculated and displayed in the Roles section.
  8. Expand the Markers panel, document the School Placement, Current Psychoactive Medication sections and record the frequency of events that occurred in the past 90 days in their respective sections.
  9. Expand the Functioning Scale panel, rate all the listed areas and then select Calculate.
    The cumulative score of the rating is calculated and displayed in the Total Score field.
  10. Optional: To send a task, select Quick Task or Send task.
  11. Select Save & Close.
    The scores of the Problem Severity, Restrictiveness of Living Environment, and Functioning Scale appear in the Screening Tools Performed grid.
