Document Adverse Childhood Experiences Screening
You can document Adverse Childhood Experiences (ACE) of patients aged between 0 to 19 years using the Adverse Childhood Experiences (ACE-Q) template.
- Open NextGen® Enterprise EHR.
- For a patient encounter, open a template.
For example, the Intake template.
- In the header link section, select Screening Tools.
The Screening Tools template opens.
- Select Adverse Childhood Experiences (ACE-Q).
The Adverse Childhood Experiences (ACE-Q) template opens. The encounter date is the default date in the Performed Date field.
- To enter a different date, select Performed Date, and then select a new date.
- To indicate that the questions are reported by the patient, select the Reported By Patient checkbox.
Note: The Reported By Patient checkbox is not available for patients aged 12 years or younger.The questions for your child are updated for you in the Additional Early Life Stressors panel.
- In the Original Ten ACE Questionnaire panel, answer all the questions.
- In the Additional Early Life Stressors panel, answer all the questions.
Note: The questions in the Additional Early Life Stressors panel are age-specific to the patient's age on the current encounter date. Questions about early life stressors are not available for patients over 19 years old.
- In the Relevant Symptomatology panel, select any checkboxes for relevant symptoms.
Note: The symptoms in the Relevant Symptomatology panel are similar to symptoms in Review of Symptoms, and are not shared with any other templates in NextGen® Enterprise EHR.
- In the Interpretation of Results section, select Calculate Score.
A two-digit score appears in Total Score. Based on the combined total score, a default interpretation appears in Interpretation.
- Optional: To override the calculated interpretation, select a new Interpretation.
Note: You can enter additional information in Comments.A note appears indicating that the interpretation is overridden.Note: When you create the document, the overridden note also appears on the document.
- Optional: To enter a referral, select the Referrals link.
- Optional: To send a task, select Quick Task or Send Task.
