Document Adverse Childhood Experiences Screening

You can document Adverse Childhood Experiences (ACE) of patients aged between 0 to 19 years using the Adverse Childhood Experiences (ACE-Q) template. Open NextGen® Enterprise EHR. For a patient encounter, open a template.For example, the Intake Plus template. In the header link section, select Screening Tools.The Screening Tools template opens. Select Adverse Childhood Experiences (ACE-Q).The Adverse Childhood Experiences (ACE-Q) template opens. The encounter date is the default date in the Performed Date field. To enter a different date, select Performed Date, and then select a new date. To indicate that the questions are reported by the patient, select the Reported By Patient checkbox. Note: The Reported By Patient checkbox is not available for patients aged 12 years or younger. The questions for your child are updated for you in the Additional Early Life Stressors panel. Adverse Childhood Experiences Screening

You can document Adverse Childhood Experiences (ACE) of patients aged between 0 to 19 years using the Adverse Childhood Experiences (ACE-Q) template. You can document the adverse childhood experiences of the patient the following panels on the Adverse Childhood Experiences (ACE-Q) template: Original Ten ACE Questionnaire panel provides questions for all ages. Additional Early Life Stressors panel consists of age-specific questions based on the patient's age as of the current encounter date. Note: Additional Early Life Stressors panel is not available for patients more than the age of 19 years. Relevant Symptomatology panel enables you to indicate any additional associated symptoms for the screening. The symptoms listed in the panel are not shared with other areas of NextGen® Enterprise EHR. The encounter date appears as the performed date by default. Interpreting ACE-Q Scores

You can calculate a two-digit score in the Interpretation of Results section on the Adverse Childhood Experiences (ACE-Q) template. The first digit of the score is the number of Yes answers in the Original Ten ACE Questionnaire panel. The second digit of the score is the number of Yes answers in the Additional Early Life Stressors panel. If you do not answer the questions in the Original Ten ACE Questionnaire panel, an application message appears indicating that the score cannot be calculated. Based on the score calculated, an interpretation of Anticipatory Guidance or Refer to Treatment appears in the Interpretation field. When the combined score in the Total Score field is 3 or less and no relevant symptomatology checkboxes are selected, the Anticipatory Guidance value appears as the Interpretation value by default. Managing Patient Details in Screening and Assessment Tools

You can document details provided by the patient in all the screening and assessment tools configured by the practice during a patient encounter. Screening tools that appear on the Screening Tools template for a selected specialty in an encounter, vary based on the configuration. When you do not select a specialty for an encounter, all the screening tools automatically appear on the Screening Tools template. You can select the Current Specialty or All option to filter the screening tools on the Screening Tools template. The following indicators appear adjacent to the screening tools and indicate whether the screening tool opens as a template or a website. An asterisk * indicates that the screening tool is used during the current encounter. The letter T indicates that the screening tool opens as a template. Remove an Adverse Childhood Experiences (ACE-Q) Screening

From a medical record template, select the Screening Tools link. The Screening Tools template opens. From the Screening Tools Performed grid, select the ACE-Q screening. Select Remove. An alert appears. To proceed with the removal, select Yes. The Adverse Childhood Experiences (ACE-Q) window opens. Select Clear All. An application message appears to confirm if you want to clear all information. Select Yes. Select Save & Close. The ACE-Q screening no longer appears in the Screening Tools Performed grid. Document the PEARLS Screening Tool

You can document pediatric patient details in the PEARLS screening tool to screen the Adverse Childhood Experiences (ACEs) and Toxic Stress. Open NextGen® Enterprise EHR. Select Specialty as Pediatrics and Visit Type as Office Visit. Expand the navigation bar, and select Screening Tools.The Screening Tools Available template opens. From the grid, select one of the following screening tools based on the patient's age group. If you are a parent or caregiver of a child of age between 0 to 11 years, select the ACEs PEARLS (0-11 years) tool. If you are a parent or caregiver of a teen of age between 12 to 19 years, select the ACEs PEARLS (12-19 years) tool.
