Document Post-traumatic Assessment Details using the PCL-5 Template

Open NextGen® Enterprise EHR. For a patient encounter, open a template. Note: Your practice can also configure the template to be accessible from thePacket Navigation (BH)template. For more information, go toNextGen Healthcare Success Community, and download theConfiguration Guide forNextGen® Behavioral Health Suite. Select the Screening Tools link. The Screening Tools template opens. Select PSTD Checklist for DSM-5 (PCL-5). Select the assessment date. Select a severity score for each question. The total score is calculated based on the selected scores for all of the questions. Select Save & Close. The information appears in the Screening Tools Performed grid on the Screening Tool window. Document General Screening Tool Information

You can use the Screening Tools template to document screenings for items that are not listed under the Screening Tools Available list. On the Screening Tools template, select a screening tool from the Screening Tools Available section. Enter the applicable information: Score Severity or interpretation Comments Major depressive disorder pre-treatment classification Plan To indicate an associated scanned document, select the See scanned document checkbox. Select Add. The documented data appears in the Screening Tools Performed grid. To print a list of screening tools during this encounter, select the notepad located next to the Screening Tools link in the template header. Document Adverse Childhood Experiences Screening

You can document Adverse Childhood Experiences (ACE) of patients aged between 0 to 19 years using the Adverse Childhood Experiences (ACE-Q) template. Open NextGen® Enterprise EHR. For a patient encounter, open a template.For example, the Intake template. In the header link section, select Screening Tools.The Screening Tools template opens. Select Adverse Childhood Experiences (ACE-Q).The Adverse Childhood Experiences (ACE-Q) template opens. The encounter date is the default date in the Performed Date field. To enter a different date, select Performed Date, and then select a new date. To indicate that the questions are reported by the patient, select the Reported By Patient checkbox. Document Serious or Traumatic Life Events

To facilitate improved trauma care, the Trauma Hx Questionnaire (BH) template provides a general questionnaire for clients to self-report on stressful incidents and trauma in their lifetime. Clients can view and manage details related to serious or traumatic life events on the Trauma Hx Questionnaire (BH) template. In NextGen® Enterprise EHR, in Patient History, select the Templates button. The Select Template window opens. On the Medical Records tab, select All. From the list, select Trauma Hx Questionnaire (BH) and select OK. The Trauma Hx Questionnaire (BH) template opens. Expand the Serious or Traumatic Life Events panel. Managing Patient Details in Screening and Assessment Tools

You can document details provided by the patient in all the screening and assessment tools configured by the practice during a patient encounter. Screening tools that appear on the Screening Tools template for a selected specialty in an encounter, vary based on the configuration. When you do not select a specialty for an encounter, all the screening tools automatically appear on the Screening Tools template. You can select the Current Specialty or All option to filter the screening tools on the Screening Tools template. The following indicators appear adjacent to the screening tools and indicate whether the screening tool opens as a template or a website. An asterisk * indicates that the screening tool is used during the current encounter. The letter T indicates that the screening tool opens as a template. The letter W indicates that the screening tool opens a website. Note: You can hide a screening tool that is configured to open as a website. For more information on screening tools l
