Document Past Medical History for Fracture Care

You can document the pre-operative information for fracture care of patients. You can use the FC Past Med Hx template in the fracture care workflow to document a patient's history and pre-surgical checklist for fracture care procedures. On the FC Past Med Hx template, click the Chief Complaint field, and select the reason for visit from the list. The lists open for selecting side of the body, body part, and complaint. Select the affected side of the body, body part, and complaint from the corresponding lists. Add or update the pharmacy, as needed. Make the desired changes in the following sections. Diagnosis Orthopedic Surgical History Allergies Medical History Surgical History Medications Social History Family History Review of Systems Note: These sections are automatically filled with the data documented on the Intake template. Document Pre-Operative History and Physical Examinations

You can review patient histories, document physical examinations, and add diagnosis details. On the Intake template, click the Non-Billable tab. The Navigation window opens. Select Pre-op H&P from the list, and click OK. The Pre-op H&P template opens. Note: The Patient is accompanied by field displays the data documented on the Intake template. You can change the value as needed. History panel on the Pre-op H and P template In the History panel, review the chief complaint, patient histories, medications, and allergies. Note: The fields in this panel are automatically filled with the data documented on the Intake template. You can document additional details or edit the details as needed. Enter the history of the present illness. In the Physical Exam panel, select an option from the left navigation bar, and document the physical examinations. Add Test Results

On the Test Results template, the following fields are automatically filled with the values added on the Patient Contact template. You can also manually enter or update the values. Taken By Stat Provider Type Who Name Return To Ext On the Home template, click the Patient Contact link, and select Test Results from the list. The Test Results template opens. Panels on the Test Results template Type the name of the clinical staff. Note: You can also double-click the Clinical Staff? field, and select the name from the list. In the Tests panel, select the tests performed and their results. In the Orders panel, do the following: Select the provider orders for the test results. Providers and Patient Histories for Pre-Registration

The Clinical 1 template enables you to add the patient histories and the details of the providers. You can use the Clinical 1 template to document the providers and shared care facilities that needs to be added in the documentation for pre-registration of a patient. You can also document any patient histories, medications, and allergies prior to the patient visit for their appointment. This template has the Providers and History panels. You can use the notes and signoff sections to add the notes and sign off the details documented on the Clinical 1 template. The Notes field is shared with the following pre-registration templates. Add Patient Contact Details

You can document a patient's contact details, generate the patient contact summary document, and generate the contact letter. You can use the Patient Contact template to document the communication details with a patient. You can also assign tasks to the provider or any other user to complete the action items. On the Home template, click the Patient Contact link, and select Patient Contact in the list. The Patient Contact template opens. Patient Contact template In the Chart Information section, enter the chart details. In the Provider Information section, click the Clinical Staff field, and select the name of the staff on the staff window. Note: The Rendering Prov field is automatically filled with the data from the login details, and the Provider field is filled while documenting the chart details. You can change the provider names. Document After Hours Contact Details

You can document the messages left and actions taken when providers are unavailable. On the Home template, click the Patient Contact link, and select After Hrs from the list. The After Hours template opens. After Hours template Click the Taken by field. Your name is automatically filled in this field, and a list opens for selecting the contact type. Select how the message was received from the list. Click the Patient of field, select the patient’s provider from the provider window, and then click OK. If the communication was with the patient, do the following: Select Patient for Who. The patient's name is automatically filled in the Caller Name field, and a list opens for selecting the phone number type. Select the phone number type. The corresponding phone number is automatically filled in the respective field. Note: You can edit the phone number, if needed.
