Creating Patient Charts

An individual added to NextGen® Enterprise PM without a chart is known as a person. A person may be someone with an existing appointment, an account guarantor, insurance subscriber, or an emergency contact. A person is allocated a unique identifier and stored in the Master Person Index (MPI) which is shared by all individuals within an enterprise system. When you create a chart for a person in a practice, the person qualifies as a patient. Only the practice that creates the patient chart, has access to it. A patient associated with multiple practices has a single demographic record that can be shared with all other practices using MPI. Person and Patient Information

In NextGen® Enterprise the initial information entered for a person or patient, such as name, birth date, address, phone, etc., is known as the "demographic record." All demographic records have a system-assigned person number and are part of the Master Person Index (MPI) in the database. This section provides information on how to add and maintain demographic records for persons and patients within the practice. Demographic information is entered when creating and updating person and patient records in NextGen® Enterprise EHR, NextGen® Enterprise PM, and NextGen Optical Management. Demographic Information in a Person or Patient Profile

On the Demographics tab of the Person Information or Patient Information window, you can enter the following data: General Information - includes name, sexual profile, social security number, and birth date. Contact Information - includes phone numbers, email address, and preferred contact methods. Demographic Information - includes marital status, student status, preferred language, religious information, and dental and primary provider. You can also enroll the patient in the Enterprise Chart feature. Insurance Information - includes insurers, authorizations, referrals, and eligibility inquiries. The optional and required fields that display on this window are determined by your system administrator's selections in Enterprise and Chart Practice Preferences. All required fields display in red text. Add Demographics to Patient Profiles

You can add demographics such as the patient's general information, address, contact information, and insurance details. You can also specify if a person is a smoker or veteran on the Demographics tab of Add Person Information window. Additionally you can enroll the patient or person in the Enterprise Chart to view encounters from different practices in NextGen® Enterprise EHR. Note: The optional and mandatory fields that appear on the Add Person Information window can be configured by your system administrator in Enterprise Preferences in File Maintenance. The mandatory fields appear in red color. Open NextGen® Enterprise PM. On the main toolbar select Lookup or People Lookup The Person Lookup window opens. Select New. Patient Chart Demographics Report

The Patient Chart Demographics report lists patients for the selected practice(s). The report sorts by practice. You can filter the report on birth date, sex, marital status, address fields, or the Chart User-defined fields. To access the report in NextGen® Enterprise PM, select the Reports menu and select General and then select Patient Chart Demographics. From the Report Filter window, select the Info button to view the following information about the report: Logic This report produces one line for each patient with a Chart in the selected Practice(s). If multiple Practices are selected, the patient will be repeated for each Practice where a chart exists. The “Rendering” column pulls data from the Default Rendering selected on the patient chart > chart details tab. Filtering Info Create date is the date the chart was created.
