On the Demographics tab of the Person Information or Patient Information window, you can enter the following data:
- General Information - includes name, sexual profile, social security number, and birth date.
- Contact Information - includes phone numbers, email address, and preferred contact methods.
- Demographic Information - includes marital status, student status, preferred language, religious information, and dental and primary provider. You can also enroll the patient in the Enterprise Chart feature.
- Insurance Information - includes insurers, authorizations, referrals, and eligibility inquiries.
The optional and required fields that display on this window are determined by your system administrator's selections in Enterprise and Chart Practice Preferences. All required fields display in red text.
