Patient Intake Details

You can document the details of a patient for the provider's reference. Document HPI for Fracture Care

You can document the history of the present illness (HPI) for fracture care. You can enter the patient's chief complaint, and access the IORTShpi Fx template from any of the following fracture care templates. Single Fx Multi Fx Poly Trauma On the appropriate fracture care template, in the History panel, enter the patient's chief complaint, and then click HPI. The IORTShpi Fx template opens displaying the chief complaint in the CC field. IORTS HPI FX template Note: The Outside Diagnostic Studies section displays the details documented on the Intake template. You can add or edit the data in this section. Enter the date of injury. In Body Area, indicate which side of the body is injured, click the blank field, and then select the injured body part from the list. To describe the injury, select the appropriate options from the following: Pain. You can click the Level field, and select the pain level from the list. Swelling. Deformity. Numbness, Tingling. Patient History

You can review, add, and update the patient histories. The History panel on the Billable Visit template enables you to select the visit type and review the diagnoses, histories, medications, allergies, and review of systems of a patient. You can also add or update the details of the history of present illness (HPI). The following grids are automatically filled with the data documented on the Intake template. You can add, edit, and remove the data as needed. Document HPI of Patients

You can document a history of present illness (HPI) for chief complaints. On the Billable Visit template, in the History panel, click on any chief complaint. The IORTShpi window opens displaying the corresponding HPI tab. IORTS HPI template Note: To document a different chief complaint, you can click the desired HPI tab. The Procedures grid displays the details from the Orthopedic Surgical History section on the Billable Visit template. Do one of the following to enter the patient's HPI. Click Get Data, select the desired encounter to copy the details from the scope window, and then click OK. In the QuickPick toolbar, click the Star icon or Double star icon icon, select the desired set name from the filter window, and then click OK. Select the appropriate options and enter the details in the fields. To add details of the patient's employer, click Add below the Employer field. Document Past Medical History for Fracture Care

You can document the pre-operative information for fracture care of patients. You can use the FC Past Med Hx template in the fracture care workflow to document a patient's history and pre-surgical checklist for fracture care procedures. On the FC Past Med Hx template, click the Chief Complaint field, and select the reason for visit from the list. The lists open for selecting side of the body, body part, and complaint. Select the affected side of the body, body part, and complaint from the corresponding lists. Add or update the pharmacy, as needed. Make the desired changes in the following sections. Additional Reason for Visit Panel Features

Additional features on the Reason for Visit panel enable you to adjust how you document chief complaints and HPI information, indicate follow-up visits, review the patient's HPI history, and remove studies reviewed from the grid. Feature Description Do not launch HPI Enables you to select one or more chief complaints/reasons for visit from the list without opening the HPI template To open the HPI template after selecting reasons for visit, click the row of one of the chief complaints listed in the grid. Additional/Manage Located in the left pane and opens the Reason for Visit template, where you can indicate a follow-up visit or you can type a chief complaint. The Reason for Visit template includes a complete list of chief complaints for the specialty Historic HPI Opens the Historic HPI template, where you can see the patient’s history of chief complaints and HPI details in a concatenated summary format for Ophthalmic specialties or all specialties Intake Comments Used by Intake staff
