Assessment and Plan Panel for Poly Trauma

The Assessment and Plan panel for Polytrauma enables the provider to submit unlimited fracture care codes. It is broken down by body area; left upper extremity, right upper extremity, spine/pelvis, left lower extremity, and right lower extremity. The grids in the top row show diagnoses/assessments, the second row of grids displays procedure codes. You can enter details for procedure orders, diagnostic tests and labs, and a management plan for each body area, as well as next visit information, X-rays, and verbal counseling. Add a Diagnosis to Plan

The Diagnosis grid displays the patient’s diagnoses. You can add a diagnosis for today’s visit or use one from the Diagnosis grid. Filter the grid by selecting the Orthopedic or Medical option above the Diagnosis grid. Click Add under the grid to launch the Common Assessment template or click Problem IT if using IMO. Select a diagnosis to add to the patient’s history. Click Save & Close. Select a diagnosis from the Diagnosis grid to add it to today’s plan. The selected diagnosis moved to the Diagnosis field. Note: For the multi-fracture template, you can select the second and third diagnosis from the Diagnosis grid and the diagnosis are assigned to the Fracture 2 and Fracture 3 field respectively. Assessment and Plan (MDM) Panel

The Assessment and Plan panel enables the provider to select the diagnosis for today’s plan, and enter today’s plan, next visit information, and verbal counseling. Assessment and Plan panel on the Non Billable Visit template The Diagnosis grid shows the patient’s diagnoses. The system defaults to the Orthopedic option to view Orthopedic-specific diagnoses. Fracture Care Workflow

Use the Fracture Care Visit templates for fracture care where an E&M code is not being calculated/billed out. There are separate templates for Single Fracture (FC Single FX) for one fracture care code, Multi FX (FC Multiple FX) for up to three fracture care codes, and Poly Trauma (FC Poly Trauma) for unlimited fracture care codes. These templates all have three panels; History, Physical Exam and Assessment and Plan. The fracture care templates are accessed by clicking the New Fracture tab and selecting the desired template. Assessment and Plan Panel- NBV Multiple FX

The Assessment and Plan panel on the NBV Multiple Fractures template enables provider to document a post-op visit for the initial fracture care procedures, the plan, verbal counseling, and next visit information. The system moves the diagnoses to the Fracture 1, Fracture 2, Fracture 3 fields in the order they are selected in the Diagnosis grid.
