Assessment and Plan Panel- NBV Multiple FX

The Assessment and Plan panel on the NBV Multiple Fractures template enables provider to document a post-op visit for the initial fracture care procedures, the plan, verbal counseling, and next visit information. The system moves the diagnoses to the Fracture 1, Fracture 2, Fracture 3 fields in the order they are selected in the Diagnosis grid. Assessment and Plan for Non-Billable Post Op Visits

The Assessment and Plan panel allows the provider to document a post-op visit for an initial fracture care procedure, the plan, verbal counseling, and next visit information. The Diagnosis grid displays the patient’s diagnoses. The system defaults to the Orthopedic option to view Orthopedic-specific diagnoses. The Medical option displays non-orthopedic diagnoses. Non-Billable Post-Op Visit Workflow

The Non-Billable Visit template is for post-op visit documentation, as well as, any non-billable visits that require HPI, Exam, and Plan information. The Non-Billable Visit template has four panels; History, Infection, Physical Exam, and Assessment and Plan (MDM). Access the Non-Billable Visit Template You can access the Non Billable Visit template from the Home template. Patient History for Post-Operative Visits You can associate surgical procedures with post-operative visits, and document a patient's history for post-operative visits. Add a Diagnosis to Plan

The Diagnosis grid displays the patient’s diagnoses. You can add a diagnosis for today’s visit or use one from the Diagnosis grid. Filter the grid by selecting the Orthopedic or Medical option above the Diagnosis grid. Click Add under the grid to launch the Common Assessment template or click Problem IT if using IMO. Select a diagnosis to add to the patient’s history. Click Save & Close. Select a diagnosis from the Diagnosis grid to add it to today’s plan. The selected diagnosis moved to the Diagnosis field. Note: For the multi-fracture template, you can select the second and third diagnosis from the Diagnosis grid and the diagnosis are assigned to the Fracture 2 and Fracture 3 field respectively. Assessment and Plan Panel (Diagnosis/MDM)

The Assessment and Plan panel has three sections to match E&M coding guidelines: Diagnosis/Management, Data Review, and Risk of complications. These allow the provider to document Medical Decision Making. The system calculates and records points for Diagnosis/Management and Data Review and assigns a level for Risk of complications based on your recorded plan to treat the patient. All of these are used to calculate the final E&M code.
