Assessment and Plan for Multiple Fracture

The Assessment and Plan panel on the FC Multiple FX enables the provider to submit up to three fracture care diagnoses codes, with procedure codes and details for each. It also allows you to document the plan for each code, verbal counseling, and next visit information. It provides quick links to cast or splint, DME, and office procedures. Add a Diagnoses for Poly Trauma

Use the top grids on the FC PolyTrauma template to add diagnoses to the specific body areas for today’s treatment. On the FC PolyTrauma template, expand the Assessment and Plan panel. Diagnoses on the FC PolyTrauma template Right-click on the grid for the body area and select Add New. Diagnoses for left lower extremity fracture care A diagnosis template opens. Each body area has its own diagnosis template. Click the Side field to enter the side of the body. Click the Fracture field to enter the fracture description. Type the ICD code in the ICD field. Fracture Care Workflow

Use the Fracture Care Visit templates for fracture care where an E&M code is not being calculated/billed out. There are separate templates for Single Fracture (FC Single FX) for one fracture care code, Multi FX (FC Multiple FX) for up to three fracture care codes, and Poly Trauma (FC Poly Trauma) for unlimited fracture care codes. These templates all have three panels; History, Physical Exam and Assessment and Plan. The fracture care templates are accessed by clicking the New Fracture tab and selecting the desired template. Add Fracture Care Procedure Codes

You can add unlimited fracture codes to each body area grid. Fracture care procedure codes on the FC Poly Trauma template Click the Procedure Codes button under the appropriate body area grid. Medical Decision Making

You can enter in the plan information. Each body area has its own plan section. The plan includes any verbal counseling done, medications, office procedures, diagnostic tests, labs, DME’s, patient education, and an area to designate follow-up visits. These fields are all picklist-driven and fully configurable within the Picklist Configuration template. You can enter data multiple different ways: Click each field to select items from configurable picklist. Click on the star or double star icons in the QuickPick Toolbar at the top of the Fracture Care template or from the MDM button that opens the MDM template to load a template default.
