Panel	Description
Care Coordination Team	This panel enables you to define the various team members who are going to provide healthcare services to a patient. When defining team member, you identify the individual provider and the agency with which the provider is associated.
Barriers to Care	This panel enables you to record and track patient circumstances that may negatively impact the patient’s ability to comply with the requirements of the nursing care plan.
History	This panel lists all the nursing care plans that have been created for a patient. These plans can be created on the History panel.
Interventions	This panel lists the current interventions that have been defined for a patient along with the relevant data necessary to define the intervention.
Progress	This panel enables you to monitor the progress of nursing care plans by reviewing the interventions.
Active and Managed Problems	The active and managed problems entered in the Comp Assessment tab are displayed in the Plan/Intervention tab.
Care Coordination	This panel indicates all providers, staff, coordinators and so on involved in a patient’s care.
Education/Recommendations	This panel enables you to identify the educational material provided to the patient.
Action Items	This panel enables you to record the actions performed during the visit.
Goals	This panel enables you to record a patient's care management goals.
Document	This panel is where documents can be generated and the Superbill can be submitted.

Documentation Tab

You use the Documentation tab to document interactions with a patient that fall outside of a patient visit. For example, interactions with other providers, telephone calls and interactions with other care team members. The Documentation tab consist of these panels: Panels Description Provider Interactions Use this panel to record interactions with other providers. Telephone Interactions Enter patient’s interactions over the phone on this panel. Face-to-Face Interactions Enter patient’s face-to-face interactions on this panel. Care Coordination Interactions Enter patient’s care coordination-related interactions on this panel. Document Use this panel to add or update the visit diagnosis codes, document the type of visit, submit the encounter visit code, and generate the visit note. The Document panel also contains links to generate several care management documents. History Panel for the Plan/Intervention Tab

The History panel lists all the nursing care plans created for a patient. Care Coordination Team Panel for the Plan/Intervention Tab

You can document about the team members who are going to provide healthcare services to a patient on the Care Coordination Team panel. Care Coordination Team The Care Coordination Team panel contains two grids that display information about the defined team members. The top Agencies grid shows information about the agencies providing the care while the bottom Interdisciplinary Team shows information about individual providers who are involved with caring for the patient. In most cases, each team member will have an entry on both grids, one on the Agencies grid for the agency or practice the member is associated with and an entry on the Interdisciplinary Team grid for the actual team member. However, it is not uncommon to have agencies defined without having a specific provider identified or a provider who is not associated with a specific agency. Comp Assessment Panels

The Comp (Comprehensive) Assessment template consists of the following panels: Panel Description General Panel The General panel displays the information documented in the Care Transition template. Recent ER Visits Panel Any recent emergency room visits by the patient are recorded on this panel. Recent Hospitalizations Panel If the patient had experienced any recent hospitalizations, then you would enter that information on this panel. Problem List Panel The Problem List panel displays a list of the patient's chronic and secondary problems as documented in the Problem List after Discharge panel on the Care Transition template. Active and Manage Problems The Active and Managed Problems panel is where active and managed problems are entered and monitored. Identify Educational Material or Make Recommendations

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. On the Plan/Intervention tab, expand the Education/Recommendations panel. Education/Recommendations Select the appropriate checkbox to identify the topic discussed with the patient. If the patient is to follow up with a care manager, select the Follow up w/ Care Manager advised checkbox. Patient instruction options appear. If the patient is to contact the care manager, select the patient to set up option.If the care manager is to contact the patient, select the Care manager to contact patient option. Add a Nursing Care Plan Data

In the nursing care plan data, you can provide details of the identified problem, goal and outcome. Open NextGen® Enterprise EHR. Access the Care Coordination Home Template. From the Nursing Care Plan History panel, select Add.The Nursing Care Plan Data template opens. Nursing Care Plan Data In the following fields, type the appropriate text or select Common Phrases and select a phrase. Problems Goals Outcome Intervention Note: For the Problem, Goals and Outcome fields, you can also select Previous Problem, Previous Goals, or Previous Outcomes and select an entry from previously saved information. Select the Status field and select the current status. Provide the Start date, Next team review date and Next intervention due date. You can select a date from the displayed calendar. The system populates the Team Member field with the name of the default provider. Note: To select another team member, select the Team Member field and select the team member who is responsible for the current i
