History Panel for the Plan/Intervention Tab
The History panel lists all the nursing care plans created for a patient. The nursing care plans can be created on the following locations: Nursing Care Plan History panel on the Care Coordination Home page Nursing Care Plan History panel on the Comp Assessment tab History panel on the Plan/Intervention tab Nursing Care Plan History The History panel for the Plan/Intervention tab functions exactly like the Nursing Care Plan History panel for the Care Coordination Home Page. Barriers to Care Panel for the Plan/Intervention Tab
You can record and track patient circumstances that may negatively impact the patient’s ability to comply with the requirements of the nursing care plan using the Barriers to Care panel. The Barriers to Care panel on the Plan/Intervention tab functions exactly like the Barriers to Care panel on the Care Coordination Home Page. Add a Document Entry
Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Document panel. Figure 1. . Document panel Select the Diagnosis field and use the resulting search screen to select the appropriate diagnosis. Select the appropriate Type of visit checkbox. If you provided a nursing care plan to the patient or family, select the Nursing care plan given to patient/family/care giver checkbox. Select Add. The system generates an entry on the data grid. Remove a Document Entry
Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Document panel. Use the Document Panel to Complete a Visit
Before you begin Ensure that you have selected the Specialty as Care Management and Visit Type as Case & Consent. Open NextGen® Enterprise EHR. On the Documentation template, expand the Document panel. Document Add or update the diagnosis code as needed. Choose the appropriate type of visit for the encounter. Select the Add button. Documentation Tab
You use the Documentation tab to document interactions with a patient that fall outside of a patient visit. For example, interactions with other providers, telephone calls and interactions with other care team members. The Documentation tab consist of these panels: Panels Description Provider Interactions Use this panel to record interactions with other providers. Telephone Interactions Enter patient’s interactions over the phone on this panel. Face-to-Face Interactions Enter patient’s face-to-face interactions on this panel. Care Coordination Interactions Enter patient’s care coordination-related interactions on this panel. Document Use this panel to add or update the visit diagnosis codes, document the type of visit, submit the encounter visit code, and generate the visit note. The Document panel also contains links to generate several care management documents.
