Document Patient Goals

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. You can record a patient's care management goals on the Goals panel. You can add, edit and remove goals. Goals Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Goals panel. Select the Goals reviewed/updated option.The Goal action options appear. If setting new goals, select the Set new goal(s) option.If the patient's care management goals have been met, select the Met goals checkbox. If patient's goals have been modified, select the Revised goal(s) checkbox. Record all the information that applies to the goal. Enter Goal statements and select Add.The collected record appears in the grid below. Do the following in the grid: To update a goal, highlight the corresponding goal, make changes, and select Update. Identify Care Coordination Providers

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. All providers, staff, coordinators and so on involved in a patient’s care are indicated in this panel. Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Care Coordination panel. Select the required option. Enter comments in the Care Coordination panel. Action Items

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. You can view the list of actions performed during the visit in the Action Items panel. Plan and Intervention Tab

You can define various team members who are going to provide healthcare services to a patient and maintain patient circumstances using the Plan/Intervention tab. The Plan/Intervention tab consists of the following panels: Panel Description Care Coordination Team This panel enables you to define the various team members who are going to provide healthcare services to a patient. When defining team member, you identify the individual provider and the agency with which the provider is associated. Barriers to Care This panel enables you to record and track patient circumstances that may negatively impact the patient’s ability to comply with the requirements of the nursing care plan. History This panel lists all the nursing care plans that have been created for a patient. These plans can be created on the History panel. Document Referrals

You can document a referral to a specialty, obstetrics, and therapy for a patient in the Referrals Order template. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. Open the Intake template, select the Family Practice specialty and Office Visit visit type, and then select My Plan.The My Plan window opens. Select the Referrals tab. The Referrals Order template opens. You can select the information icon to learn more about the Referrals Order template. On the Referrals tab, select the insurance provider. Enter the insurance policy number, and then select the referral type. In the To section, select the specialty.For example, select DME to add a referral for the durable medical equipment. Note: You can select Exclusions to document exceptions. For Authorization, select Yes or No. In the Diagnosis section, select the diagnoses. Enter Counseling Details

You can enter the counseling details for a patient and include them in the visit note. Open NextGen® Enterprise EHR. Open a patient record. Open the Finalize template. On Evaluation and Management Coding, select the Counseling Details link.The Counseling Details template opens. In the Counseling/Educational Details section, do the following: Select the counseling type, method, and evaluation.The counselor's name and date appear. Enter the time spent in patient counseling. Enter additional counseling information. Enter the type of educational materials the patient received. To enter details about an interpreter, select the corresponding checkbox, and then enter their language, name, and relationship with the patient. You can enter details for up to five types of counseling and education. Total counseling time (minutes) displays the sum of the total time entered for counseling sessions you entered.
