Identify Care Coordination Providers
Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. All providers, staff, coordinators and so on involved in a patient’s care are indicated in this panel. Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Care Coordination panel. Select the required option. Enter comments in the Care Coordination panel. Add an Action Item
Before you begin Ensure that you have selected the Specialty as Care Management and Visit Type as Case & Consent. Open NextGen® Enterprise EHR. On the Tracking template, expand the Action Item List panel, and select Add. Select Add.The Care Management Tasks template opens. New Record Window The Time, Date and Staff fields are populated with current information. Select the Due date field and select a date from the calendar. Select the Priority field and select a priority from the list. Type any relevant information about the task in the Details field. Select the Status field and select the status from the list. Note: Selecting the Task button opens the Attachment, then the Select Recipients windows to send an task. Select Clear For Add, and then select Save or Close. Parent topic: Tracking Tab

Remove a Document Entry
Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Document panel. Add a Document Entry
Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Document panel. Figure 1. . Document panel Select the Diagnosis field and use the resulting search screen to select the appropriate diagnosis. Select the appropriate Type of visit checkbox. If you provided a nursing care plan to the patient or family, select the Nursing care plan given to patient/family/care giver checkbox. Select Add. The system generates an entry on the data grid. Complete an Intervention
Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. When the goal of an intervention has been achieved, you can complete the intervention. Completing the intervention removes it from the Interventions grid. Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Interventions panel. Select an entry on the Interdisciplinary Team Member column.The system displays the interventions designated for the selected team member type in the grid. Select the intervention that you want to update and select Edit.The Nursing Care Plan Data template appears. The template is populated with the data for the selected intervention. Select the Intervention complete option. Change any other enabled fields as needed. Select Save & Close.The intervention is completed and removed from the intervention panel. Use the Document Panel to Complete a Visit
Before you begin Ensure that you have selected the Specialty as Care Management and Visit Type as Case & Consent. Open NextGen® Enterprise EHR. On the Documentation template, expand the Document panel. Document Add or update the diagnosis code as needed. Choose the appropriate type of visit for the encounter. Select the Add button.
