Document Patient Goals

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. You can record a patient's care management goals on the Goals panel. You can add, edit and remove goals. Goals Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Goals panel. Select the Goals reviewed/updated option.The Goal action options appear. If setting new goals, select the Set new goal(s) option.If the patient's care management goals have been met, select the Met goals checkbox. If patient's goals have been modified, select the Revised goal(s) checkbox. Record all the information that applies to the goal. Enter Goal statements and select Add.The collected record appears in the grid below. Do the following in the grid: To update a goal, highlight the corresponding goal, make changes, and select Update. Action Items

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. You can view the list of actions performed during the visit in the Action Items panel. Action Items Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Action Items panel. Select the corresponding actions performed. Enter Action Items/Follow up comments. Care Coordination Team Panel for the Plan/Intervention Tab

You can document about the team members who are going to provide healthcare services to a patient on the Care Coordination Team panel. Care Coordination Team The Care Coordination Team panel contains two grids that display information about the defined team members. The top Agencies grid shows information about the agencies providing the care while the bottom Interdisciplinary Team shows information about individual providers who are involved with caring for the patient. In most cases, each team member will have an entry on both grids, one on the Agencies grid for the agency or practice the member is associated with and an entry on the Interdisciplinary Team grid for the actual team member. However, it is not uncommon to have agencies defined without having a specific provider identified or a provider who is not associated with a specific agency. Identify Educational Material or Make Recommendations

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. On the Plan/Intervention tab, expand the Education/Recommendations panel. Education/Recommendations Select the appropriate checkbox to identify the topic discussed with the patient. If the patient is to follow up with a care manager, select the Follow up w/ Care Manager advised checkbox. Patient instruction options appear. If the patient is to contact the care manager, select the patient to set up option.If the care manager is to contact the patient, select the Care manager to contact patient option. Access the Care Coordination Home Template

Open NextGen® Enterprise EHR. On the Top Toolbar, select Templates icon. To view all the available templates, select All. On the Medical Records tab, search for and select Care Coordination Home. Record Care Coordination Interactions

Before you begin Ensure that you have selected the Specialty as Care Management and Visit Type as Case & Consent. Open NextGen® Enterprise EHR. On the Documentation template, expand the Care Coordination Interactions panel, and then select Add.The Care Coordination Interactions template opens. Fill out all the fields that apply for this interaction. Select Save. Select Clear For Add to enter multiple actions or select Close.
