From the
Interdisciplinary Team
section, select
Add
.
The
Care Coordination Team
template opens.
Type the provider’s name in the
Name
field.
From the
Interdisciplinary team role
field, select the provider’s role.
This role is specified for interventions and is reported on several templates and enables you to track the progress of a nursing care plan by role.
Type the address of the provider in the
Address
field.
Type the phone number of the provider in the
Phone
field.
Do one of the following:
To save the current entry and to add another team member, select
Save & Add New
.
To save the current entry and to close the template, select
Save and Close
.
The team member is added to the
Interdisciplinary Team
grid.
Note:
You can add an agency and a team member at the same time. If you add both, entries appear on both grids.

Define Agencies
Open NextGen® Enterprise EHR. Select Add for the Agencies grid.The Care Coordination Team template opens. Care Coordination Team Type the agency type in the Agency type field. The type can be any text that you choose but should meaningfully describe the type of services that the agency provides, such as pain management, dialysis, physical therapy, and so on. Type the agency’s name in the Agency name field. Type the agency’s phone number and fax number in the Agency phone and Agency fax fields respectively. If you want to add another agency, select the Save & Add New button.The system saves your entries and allows you to enter data for another team member. If you are finished adding agencies, select Save and Close.The system closes the template. An entry for the new agency appears on the Agencies grid. Update an Agency or Team Member
Open NextGen® Enterprise EHR. Select to highlight the agency in the Agencies section or the team member in the Interdisciplinary Team section that you want to update. Select Edit for either section.The Care Coordination Team template appears populated with data for the selected agency or team member. Update the fields as required. You can change the information for either the agency or the team member or both, regardless of which Edit button you clicked. Select Save & Close.You are returned to your originating template with the team member or agency updated on the Agencies or Interdisciplinary team grid. Remove an Agency or Team Member
You can remove an agency or team member at any time. When you remove an agency, any associated team member entry is also removed and when you remove a team member, any associated agency entry is removed also. Open NextGen® Enterprise EHR. Select to highlight the agency or member on the Care Coordination Team panel that you want to delete. Select Remove.You are prompted to confirm the delete. Select Yes.You are returned to your originating template with the selected barrier to care removed from the Agencies or Interdisciplinary team grid. Care Coordination Team Panel For the Care Coordination Home Page
The team members who are going to provide healthcare services to a patient must be defined on the Care Coordination Team panel. Care Coordination Team The Care Coordination Team panel contains two grids that display information about the defined team members. The top Agencies grid shows information about the agencies providing the care while the bottom Interdisciplinary Team shows information about individual providers who are involved with caring for the patient. In most cases, each team member will have an entry on both grids, one on the Agencies grid for the agency or practice the member is associated with and an entry on the Interdisciplinary Team grid for the actual team member. However, it is not uncommon to have agencies defined without having a specific provider identified or a provider who is not associated with a specific agency. Add a Nursing Care Plan Data
In the nursing care plan data, you can provide details of the identified problem, goal and outcome. Open NextGen® Enterprise EHR. Access the Care Coordination Home Template. From the Nursing Care Plan History panel, select Add.The Nursing Care Plan Data template opens. Nursing Care Plan Data In the following fields, type the appropriate text or select Common Phrases and select a phrase. Problems Goals Outcome Intervention Note: For the Problem, Goals and Outcome fields, you can also select Previous Problem, Previous Goals, or Previous Outcomes and select an entry from previously saved information. Select the Status field and select the current status. Provide the Start date, Next team review date and Next intervention due date. You can select a date from the displayed calendar. The system populates the Team Member field with the name of the default provider. Update an Intervention
Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. The interventions appear only if they are added in the Nursing Care Plan Data template in the History panel. Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Interventions panel. Select an appropriate entry from the Interdisciplinary Team Member column.The system displays the interventions designated for the selected team member type. Interventions Select the intervention that you want to update and select Add.The Nursing Care Plan Data template opens. The template is populated with the data for the selected intervention.
