Edit an Existing Nursing Care Plan

The existing nursing care plan information can be updated when required. Open NextGen® Enterprise EHR. Access the Care Coordination Home Template. Expand the Nursing Care Plan History panel. From the Nursing Care Plan History panel, select the record that you want to update. Select Edit.The Nursing Care Plan Data template opens. Update the information on the Nursing Care Plan Data template following the instructions given for adding a nursing care plan. To save the updates, select Save & Close. Update an Intervention

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. The interventions appear only if they are added in the Nursing Care Plan Data template in the History panel. Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Interventions panel. Select an appropriate entry from the Interdisciplinary Team Member column.The system displays the interventions designated for the selected team member type. Interventions Select the intervention that you want to update and select Add.The Nursing Care Plan Data template opens. The template is populated with the data for the selected intervention. Add a Patient Goal to Address a Health Concern

You can add multiple goals to a single health concern. To add multiple goals, repeat this procedure for each goal. Open NextGen® Enterprise EHR. Expand the Goals, Interventions and Outcomes panel. The Select a Health concern to document section appears with all previously added health concerns. Goals, Interventions and Outcomes panel Select a health concern from the list. The information about the selected health concern goal fields appears on the right. Goal details on the Goals, Interventions and Outcomes panel In the Goal field, type a goal description to address the health concern, or use Common Phrases. Indicate if the goal is a patient goal or a provider goal. Verify that the life cycle status of the goal is set to Active. You can also change the status. Complete an Intervention

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. When the goal of an intervention has been achieved, you can complete the intervention. Completing the intervention removes it from the Interventions grid. Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Interventions panel. Select an entry on the Interdisciplinary Team Member column.The system displays the interventions designated for the selected team member type in the grid. Select the intervention that you want to update and select Edit.The Nursing Care Plan Data template appears. The template is populated with the data for the selected intervention. Select the Intervention complete option. Change any other enabled fields as needed. Select Save & Close.The intervention is completed and removed from the intervention panel. Record an Outcome for an Intervention

Open NextGen® Enterprise EHR. Open to the Goals, Interventions and Outcomes panel. The Select a Health concern to document grid is populated with all previously identified health concerns. Select a health concern from the list. The selected health concern appears in the Health Concern field and the data entry fields for goals appear to the right. Select the goal that you are adding an intervention outcome for. The data for the selected goal fills the data entry fields to the right. Select the Interventions tab. The full Interventions tab appears with the previously entered interventions in the Select an Intervention to document grid. Select the intervention in the grid that you are reporting an outcome for. Select the Outcomes tab. The full Outcomes tab appears with the data entry fields for the outcome to the right. Update an Intervention Outcome

You can modify all fields for an outcome entered during the current encounter. Since outcomes are encounter specific, you cannot view outcomes entered during previous encounters in the Goals, Interventions and Outcomes panel and therefore cannot modify those outcomes. You can view outcomes from previous encounter in the Care Plan History panel. Open NextGen® Enterprise EHR. Open to the Goals, Interventions and Outcomes panel. Select the goal you what to update an intervention outcome. The system fills the fields to the right of the grid with the data from the selected record. Select the Intervention tab. The full Intervention tab appears with the previously entered interventions in the grid. Select the intervention with an outcome to be updated. Select the Outcomes tab. The full Outcomes tab appears with the data for the selected intervention. Make any necessary changes to the data fields. Select Update. The system updates the outcome.
