Add a Patient Goal to Address a Health Concern

You can add multiple goals to a single health concern. To add multiple goals, repeat this procedure for each goal. Open NextGen® Enterprise EHR. Expand the Goals, Interventions and Outcomes panel. The Select a Health concern to document section appears with all previously added health concerns. Goals, Interventions and Outcomes panel Select a health concern from the list. The information about the selected health concern goal fields appears on the right. Goal details on the Goals, Interventions and Outcomes panel In the Goal field, type a goal description to address the health concern, or use Common Phrases. Indicate if the goal is a patient goal or a provider goal. Verify that the life cycle status of the goal is set to Active. You can also change the status. For Goal achieved, do one or more of the following: Select Yes, No, Goal not attainable, or No progress toward goal. Select the field below and then select an option from the list that appears. Create an Intervention to Achieve a Goal

You can add multiple interventions to a single goal. To add multiple interventions, repeat this procedure for each intervention to be added. Open NextGen® Enterprise EHR. Open to the Goals, Interventions and Outcomes panel.The Select a Health concern to document grid is populated with all previously identified health concerns. Select a health concern from the list.The selected health concern appears in the Health Concern field and the data entry fields for goals appear to the right. Select the goal that you are adding an intervention for.The selected goal appears in the Goal field and data for the selected goal fills the data entry fields to the right. Select the Interventions tab.The full Interventions tab appears. Interventions tab on the Goals, Interventions and Outcomes panel The selected goal appears in the Goal field. Update an Intervention Outcome

You can modify all fields for an outcome entered during the current encounter. Since outcomes are encounter specific, you cannot view outcomes entered during previous encounters in the Goals, Interventions and Outcomes panel and therefore cannot modify those outcomes. You can view outcomes from previous encounter in the Care Plan History panel. Open NextGen® Enterprise EHR. Open to the Goals, Interventions and Outcomes panel. Select the goal you what to update an intervention outcome. The system fills the fields to the right of the grid with the data from the selected record. Select the Intervention tab. The full Intervention tab appears with the previously entered interventions in the grid. Select the intervention with an outcome to be updated. Select the Outcomes tab. The full Outcomes tab appears with the data for the selected intervention. Make any necessary changes to the data fields. Select Update. The system updates the outcome. Remove an Intervention Outcome

You can remove an outcome only in the encounter where the latest outcome was documented for that intervention. Open NextGen® Enterprise EHR. Open the patient encounter where the latest outcome for the intervention was documented. Open to the Goals, Interventions and Outcomes panel. Select the goal you what to remove an intervention outcome.The system fills the fields to the right of the grid with the data from the selected record. Select the Intervention tab.The full Intervention tab appears with the previously entered interventions in the grid. Select the intervention with an outcome to be removed. Select the Outcomes tab.The full Outcomes tab appears with the data for the selected intervention. Select the outcome to be removed from the grid. Select Remove.The system removes the outcome. Update a Goal Intervention

If no outcome has been entered, you can update all Intervention data fields for the current encounter. If an Outcome has been entered, you cannot update the Category field and the related data field. If updating interventions entered during earlier encounters, you may only be able to update some of the fields. You receive an alert to that effect. To update those fields, you must return to the earlier encounter. Open NextGen® Enterprise EHR. Open to the Goals, Interventions and Outcomes panel. Select the goal you what to update an intervention. The system fills the fields to the right of the grid with the data from the selected record. Select the Intervention tab. The full Intervention tab appears with the previously entered interventions in the grid. Select the intervention to be updated. Make any necessary changes to the data fields. Select Update. Add a Nursing Care Plan Data

In the nursing care plan data, you can provide details of the identified problem, goal and outcome. Open NextGen® Enterprise EHR. Access the Care Coordination Home Template. From the Nursing Care Plan History panel, select Add.The Nursing Care Plan Data template opens. Nursing Care Plan Data In the following fields, type the appropriate text or select Common Phrases and select a phrase. Problems Goals Outcome Intervention Note: For the Problem, Goals and Outcome fields, you can also select Previous Problem, Previous Goals, or Previous Outcomes and select an entry from previously saved information. Select the Status field and select the current status. Provide the Start date, Next team review date and Next intervention due date. You can select a date from the displayed calendar.
