Document Patient Goals
Before you begin
Ensure that you have selected the
Specialty
and
Visit Type
as
Care Management
.
You can record a patient's care management goals on the
Goals
panel. You can add, edit and remove goals.
Open
NextGen® Enterprise EHR
.
On the
Plan/Intervention
tab, expand the
Goals
panel.
Select the
Goals reviewed/updated
option.
The Goal action options appear.
If setting new goals, select the
Set new goal(s)
option.
If the patient's care management goals have been met, select the
Met goals
checkbox.
If patient's goals have been modified, select the
Revised goal(s)
checkbox.
Record all the information that applies to the goal.
Enter
Goal statements
and select
Add.
The collected record appears in the grid below.
Do the following in the grid:
To update a goal, highlight the corresponding goal, make changes, and select
Update
.
To remove a goal, highlight the goal to be removed, and select
Remove
.

Add a Patient Goal to Address a Health Concern
You can add multiple goals to a single health concern. To add multiple goals, repeat this procedure for each goal. Open NextGen® Enterprise EHR. Expand the Goals, Interventions and Outcomes panel. The Select a Health concern to document section appears with all previously added health concerns. Goals, Interventions and Outcomes panel Select a health concern from the list. The information about the selected health concern goal fields appears on the right. Goal details on the Goals, Interventions and Outcomes panel In the Goal field, type a goal description to address the health concern, or use Common Phrases. Indicate if the goal is a patient goal or a provider goal. Verify that the life cycle status of the goal is set to Active. You can also change the status. Enter New Therapy Goals for the Patient
Therapy goals are based on information documented for the reason for visit, History of Present Illness (HPI), and physical exam for the patient. Preconfigured therapy goals configurations are packaged with the system. Clients can add customized goals and modify or remove existing ones. Goals can be added, updated or removed. Open NextGen® Enterprise EHR. On the Objective template, on the Goals panel, click the Add button to launch the Therapy Goals template, which displays accumulated goals listed in the Available Goals grid. Therapy Goals Highlight a row in the Available Goals grid to select a goal. Click the Goal type field and select the type of goal from the picklist.The remaining fields display additional details for the goal. Click the Weeks field and select the number of weeks to reach the goal, if appropriate. Click Add.The Added goals grid displays the new goal. Click Save & Close to return to the Objective template. Remove a Goal
Open NextGen® Enterprise EHR. On the Objective template, highlight the goal to be deleted in the Goals panel grid. Click Remove.The grid no longer displays the selected goal. Parent topic: Patient Goals

Configure Goals for a Recommended Care Guideline
You can set up multiple goals for a patient's Recommended Care guideline. Open NextGen® Enterprise EHR. From File, select System \ Practice Template. The Select Template window opens. On the Practice tab, select the All option. Select Recommended Care, and then select OK.The Recommended Care practice template opens. Select a row in the data grid to add a goal.The associated goal, status, and compliance details appear in the corresponding grids. Select the goal from the list, select OK, and then select Add.The goal is added to the data grid. To delete a goal, select a row in the data grid, and then select Delete. Enter, Review, and Update Guideline Status
You can document the details for the patient on the Care Guidelines grid view template. Open NextGen® Enterprise EHR. Select the Care Guidelines link on any of the main template. The Care Guidelines Paged View template opens. In the Care Guidelines panel, select the Grid View link. The Care Guidelines grid view template opens. Select a guideline item from the grid. Details for the care guideline fill in the fields. To edit the changes for information appeared for the guideline, do the following: In the Goal field, enter a goal for the patient or select from the list. Select the Status from the list. Select the Action from the list. Note: For guideline items that are ordered, fills in automatically. The Status and Action fields are read-only. Care Guidelines Select Update. The grid displays the updated Status. Note: To see the goal, select the item in the grid to display details in the fields. Create a Care Plan
Before you begin Make sure that the existing care plans for the patient are in Completed status. Open NextGen® Enterprise EHR. From the navigation, select Care Plan.The Care Plan template appears. Care Plan template Select Create a New Care Plan.All the panels gets activated. Activated panels on the Care Plan template Select a status, and an intent.The default values are Active, and Plan respectively. Add a health concern. Add goals to address health concern. Add interventions to achieve the goals. Record functional and cognitive assessments. Select Save and Close. Note: To edit a completed care plan, you must update the status of the current plan to Entered-in-Error, which removes the current care plan's data and redirects you to the previous care plan.
