Care Plan Components

You can create a care plan to address a patient's health concerns and to monitor their progress toward resolving the identified concerns on the Care Plan template. The care plan consists of following components: Field Description Type The type of care plan Status The status of the care plan Intent The intent of the care plan Panel Description Add/Edit Health Concerns Consolidates categorized health concerns from several sources within the application Health concern categories include but are not limited to allergy, vital signs, social history, smoking and tobacco use. Patient Care Team Members Displays information about a patient's care team and enables providers to update the care team by adding modifying members Functional and Cognitive Assessment Status Enables providers to record functional and cognitive assessments for patients Goals, Interventions, and Outcomes Enables providers to define goals, identify interventions, and track outcomes for identified patient health concerns Car

Add a Patient Goal to Address a Health Concern

You can add multiple goals to a single health concern. To add multiple goals, repeat this procedure for each goal. Open NextGen® Enterprise EHR. Expand the Goals, Interventions and Outcomes panel. The Select a Health concern to document section appears with all previously added health concerns. Goals, Interventions and Outcomes panel Select a health concern from the list. The information about the selected health concern goal fields appears on the right. Goal details on the Goals, Interventions and Outcomes panel In the Goal field, type a goal description to address the health concern, or use Common Phrases. Indicate if the goal is a patient goal or a provider goal. Verify that the life cycle status of the goal is set to Active. You can also change the status. For Goal achieved, do one or more of the following: Select Yes, No, Goal not attainable, or No progress toward goal. Select the field below and then select an option from the list that appears. Care Plan Statuses

You can view or update a care plan status in the Care Plan template. You can set the status as Draft, Active, On-hold, Revoked, Completed, Entered-in-Error, or Unknown. The default status is set to Active. You can update the care plan status to Completed only when the plan meets the following criteria: Health concerns are in inactive, remission, or resolved Goals are completed, canceled, entered in error, or rejected Interventions are changed, completed, or discontinued When you set the care plan status as Completed, all the field values (except Status, and Intent) corresponding to the care plan are disabled. When you change the care plan status from Completed to Active, you can edit the existing plan. You can only create a care plan, when existing plan is completed. When you set the care plan status as Entered in Error, all associated health concerns, goals, interventions, and outcomes are removed and you are redirected to the previous care plan. Patient Care Plans

You can create a care plan to address patients' health concerns and monitor their progress toward resolving specific concerns. On the Care Plan template, you can do the following: Define and maintain a care plan for a patient that addresses health concerns, goals, interventions, and outcomes Record the functional and cognitive status of the patient Define, track, and review interventions by multiple providers and agencies Track the overall progress of the patient toward achieving defined goals The Care Plan template also enables you to meet the 2015 edition certification criterion, which requires a recording, changing, accessing, creating, and receiving care plan information. You can also create a Care Plan document from the Care Plan template and save the document as part of the encounter history. Document Functional and Cognitive Assessments

You can document a patient's functional and cognitive assessments in the Functional and Cognitive Assessment Status panel. The data entered in the Functional and Cognitive Assessment Status panel is shared with the Health Promotion Plan, Medicare Preventive HPI, Screening Summary, and Care Plan templates to measure compliance and for surveillance reporting. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel, and then select the Health Promotion Plan link. The Health Promotion Plan template opens. Expand the Functional and Cognitive Assessment Status panel. Functional and Cognitive Assessment Status For Functional Status and Cognitive Status, do the following: If there is no change in the functional status of the patient, select No Change. If there is a change in the functional status or cognitive status of the patient, select Change. Select the Status field, and select the appropriate option from the list. Create an Intervention to Achieve a Goal

You can add multiple interventions to a single goal. To add multiple interventions, repeat this procedure for each intervention to be added. Open NextGen® Enterprise EHR. Open to the Goals, Interventions and Outcomes panel.The Select a Health concern to document grid is populated with all previously identified health concerns. Select a health concern from the list.The selected health concern appears in the Health Concern field and the data entry fields for goals appear to the right. Select the goal that you are adding an intervention for.The selected goal appears in the Goal field and data for the selected goal fills the data entry fields to the right. Select the Interventions tab.The full Interventions tab appears. Interventions tab on the Goals, Interventions and Outcomes panel The selected goal appears in the Goal field. The Category defaults to Procedure and the data entry field below defaults to Procedure. If necessary, select a different Category.The data entry field changes base
