Consolidating Care Team Member Information

You can consolidate a list of care team members for a patient on the Care Team Members template. The patient care team can include clinical professionals, clinical pharmacist, agencies, organizations, relatives who act as care givers, and any other individual involved directly or indirectly with the care of the patient. These consolidated details in the Care Team Members template provide a comprehensive and up-to-date list of a patient’s care team members and meet the Stage 3 Quality Reporting Certification for 2017. The list includes patient care team members who are entered throughout NextGen® Adaptive Content Engine and the members added using the NPI Registry Search window. However, members added using the Care Team Members template only appear on the Care Team Members template and are shared with the Patient Care Team panel on the Care Plan template. Patient Care Team Members

You can use the Patient Care Team Members panel to view and manage a list of providers, agencies, organizations, care takers, and other individuals involved in the ongoing care of the patient. You can create and update patient care team details, either from the Patient Care Team Members panel of the Care Plan template or from the Care Team Members template. From the Patient Care Team Members panel, you can: Create and update patient care team. Add, update, and remove members and their details from the patient care team. Import practice care team to patient care team as one of the member. Care Plan Components

You can create a care plan to address a patient's health concerns and to monitor their progress toward resolving the identified concerns on the Care Plan template. The care plan consists of following components: Field Description Type The type of care plan Status The status of the care plan Intent The intent of the care plan Panel Description Add/Edit Health Concerns Consolidates categorized health concerns from several sources within the application Health concern categories include but are not limited to allergy, vital signs, social history, smoking and tobacco use. Create a Care Plan

Before you begin Make sure that the existing care plans for the patient are in Completed status. Open NextGen® Enterprise EHR. From the navigation, select Care Plan.The Care Plan template appears. Care Plan template Select Create a New Care Plan.All the panels gets activated. Activated panels on the Care Plan template Select a status, and an intent.The default values are Active, and Plan respectively. Add a health concern. Add goals to address health concern. Add interventions to achieve the goals. Record functional and cognitive assessments. Select Save and Close. Note: To edit a completed care plan, you must update the status of the current plan to Entered-in-Error, which removes the current care plan's data and redirects you to the previous care plan. Patient's Treatment Plan

The treatment plan for the patient can be documented on the My Plan or My Custom Plan template. The My Plan template opens within NextGen® Adaptive Content Engine by default and can be replaced with the My Custom Plan template based on the configuration in the Practice Configuration practice template. The My Custom Plan template can be customized to suit your practice's needs and is less select-intensive.
