Open the Assessment Plan Details Template

Some specialties (for example, Cardiology) open the Assessment Plan Details template differently. Refer to the specialty guide for your version of NextGen® Adaptive Content Engine available on NextGen Healthcare Success Community for instructions if the following does not apply: Open NextGen® Enterprise EHR. On the SOAP template, in Assessment/Plan, select A/P Details.The Assessment Plan Details template opens. Document a Treatment Plan

You can document treatment plan for a patient. Note: You must enter a diagnosis for the patient before documenting a treatment plan. Open NextGen® Enterprise EHR. On the SOAP template, in the Assessment / Plan panel, select My Plan in the navigation bar.The My Plan template opens. If required, select the assessment in the Today's Assessments field.The My Plan template, which is part of a group of templates, uses the last assessment or diagnosis selected on any of the templates in the group as the working assessment. The system defaults to the current working assessment. You can select another assessment, which becomes the new working assessment that carries forward to the other templates in the group. The selected assessment appears in the Selected Dx field. The system displays the order set for the associated diagnosis category. Document the Treatment Plan on My Plan

The treatment plan for the patient can be documented on the My Plan (PT) template that enables providers to place multiple orders from a single template. The My Plan (PT) template is based on order sets for diagnosis categories that can be modified and managed by clients using practice templates. The My Plan (PT) template appears within NextGen® Adaptive Content Engine by default and can be easily replaced with the My Custom Plan template after a simple configuration in the Practice Configuration practice template. The My Custom Plan template can be personalized to suit your practice's specific needs and is less click-intensive. My Plan Versus My Custom Plan to Create a Treatment Plan

You can document a treatment plan that is personalized to suit your practice's specific needs. The My Plan tab contains the My Custom Plan template that helps you configure as a grid for order sets, replacing the need to manually select a checkbox for each order to be placed. The individualized fields available on the original My Plan template require manual selection of every order needed for a particular diagnosis. The My Custom Plan template reduces the number of selects required for ordering while maintaining the process for managing order sets and provider plans. The My Custom Plan template uses a more streamlined user interface and takes a personalized approach to diagnosis-based order sets. In the customized template workflow, the individualized fields are uploaded to a grid in the Pending Orders section. My Plan Sub-Navigational Link

The My Plan sub-navigational link opens the My Plan or My Custom Plan template to select an assessment and place the applicable orders associated with the diagnosis categories for the selected assessment. The template that opens in the My Plan tab out-of-the-box is the original template. Note: The My Plan tab can hold one of two templates: My Plan or My Custom Plan. My Plan - is packaged as the default template; recommended for those clients who wish to continue using the current My Plan template. This template will eventually be retired (date to be determined) and no enhancements are being applied to it. My Custom Plan – created to improve usability by reducing the number of selects required for ordering while maintaining the functionality for Order Sets; recommended for clients who wish to take advantage of the significant modifications in the areas of Orders and Referrals. Diabetic Treatment Plans

The My Plan template contains a preconfigured order set for diabetes. Practices can customize the order sets at the practice or provider level. This clinical decision support prompts the provider to consider certain tests and instructions for diabetic patients. Items in the Instruction section are user defined and disease specific. They give credit toward Quality Reporting education and instruction. Note: The Additional orders section can be configured to provide additional instructions or lab orders.
