Place a Follow-up or Disposition Order

Open NextGen® Enterprise EHR. Do one of the following: In the Assessment Plan panel, click the A/P Details link in the left pane. On the My Plan (PT) template, click the A/P Details tab. The Assessment Plan Details template opens. Click Follow Up.The Follow-up/Disposition template opens. Follow-up/Disposition template To add or update the working assessment, select the diagnosis in the grid. Select the follow up checkbox. Enter the details and time for the follow-up visit. In the Disposition field, select an option from the list. If the applicable option is not available in the list, click the top blank row and then type a value in the Disposition field. If the patient refused the selected disposition, select the Refused checkbox. In the Reason field, select an option from the list. In the Timeframe field, select an option from the list. Click Place Order.The fields clear so that another follow-up appointment can be entered. The follow-up appointments appear in the Assessment/Plan grid

Enter Additional Assessment and Plan Details

You can document additional information such as a differential diagnosis, plan details, or comments for the assessment on the Assessment/Plan Details template. On the My Plan (PT) template, click the A/P Details tab.The Assessment Plan Details template opens. Assessment Plan Details To add or update the working assessment, select an assessment in the Today's Assessments grid. Type or dictate patient data in the Impression/Comments, Plan Details, Differential Diagnosis, and Provider Details. For a follow-up visit or disposition is needed, place the order. Patient's Treatment Plan

The treatment plan for the patient can be documented on the My Plan or My Custom Plan template. The My Plan template opens within NextGen® Adaptive Content Engine by default and can be replaced with the My Custom Plan template based on the configuration in the Practice Configuration practice template. The My Custom Plan template can be customized to suit your practice's needs and is less select-intensive. My Plan Versus My Custom Plan to Create a Treatment Plan

You can document a treatment plan that is personalized to suit your practice's specific needs. The My Plan tab contains the My Custom Plan template that helps you configure as a grid for order sets, replacing the need to manually select a checkbox for each order to be placed. The individualized fields available on the original My Plan template require manual selection of every order needed for a particular diagnosis. The My Custom Plan template reduces the number of selects required for ordering while maintaining the process for managing order sets and provider plans. The My Custom Plan template uses a more streamlined user interface and takes a personalized approach to diagnosis-based order sets. In the customized template workflow, the individualized fields are uploaded to a grid in the Pending Orders section. Document a Patient's Custom Plan

You can document the patient's diagnostics, insurance, lab orders, diagnostic, referrals, and follow-up details in the My Custom Plan template. Before you begin On My Custom Plan practice template, in the General Configuration panel, select the Launch My Custom Plan checkbox. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. Open the Intake template, select the specialty as Family Practice and the visit type as Office Visit, and then select My Plan.The My Custom Plan window opens. Add Assessments or Plans

Open NextGen® Enterprise EHR. Open the SOAP template. Click the Assessment/Plan panel. From the left pane, click an assessment category link.The diagnosis search window displays a list of diagnoses for the category selected. Select an appropriate diagnosis and then click Select.The diagnosis has been added to and displayed in the grid with the letter “D” and lines for A and P.
