Place a Follow-up or Disposition Order

Do one of the following:
In the
Assessment Plan
panel, click the
A/P Details
link in the left pane.
On the
My Plan (PT)
template, click the
A/P Details
tab.
The
Assessment Plan Details
template opens.
Click
Follow Up
.
The
Follow-up/Disposition
template opens.
To add or update the
working assessment
, select the diagnosis in the grid.
Select the
follow up
checkbox.
Enter the details and time for the follow-up visit.
In the
Disposition
field, select an option from the list.
If the applicable option is not available in the list, click the top blank row and then type a value in the
Disposition
field.
If the patient refused the selected disposition, select the
Refused
checkbox.
In the
Reason
field, select an option from the list.
In the
Timeframe
field, select an option from the list.
Click
Place Order
.
The fields clear so that another follow-up appointment can be entered. The follow-up appointments appear in the
Assessment/Plan
grid.
Click
OK
.

Shared Working Assessment
The last assessment or diagnosis you select on any of the following templates carries over as the working assessment when you open another template in the group: Add or Update Assessment Assessment Plan Details Diagnostic Studies Chemistry Lab Hematology Lab Master Microbiology Other Labs Add or Update Assessment Therapeutic Levels Serology Tumor markers Add or Update Assessment Urine Chemistry Office Procedures My PlanNote: The My Custom Plan template and My Plan (PT) template are not part of the template group that shares the working assessment. Document the Treatment Plan on My Plan
The treatment plan for the patient can be documented on the My Plan (PT) template that enables providers to place multiple orders from a single template. The My Plan (PT) template is based on order sets for diagnosis categories that can be modified and managed by clients using practice templates. The My Plan (PT) template appears within NextGen® Adaptive Content Engine by default and can be easily replaced with the My Custom Plan template after a simple configuration in the Practice Configuration practice template. The My Custom Plan template can be personalized to suit your practice's specific needs and is less click-intensive. Capture the Follow-up and Disposition Details for an Assessment
Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. Open the Intake template, select the specialty as Family Practice and the visit type as Office Visit, and then select My Plan. The My Plan window opens. Select the A/P Details tab and then select Follow Up.The Follow-up/Disposition template window opens. Follow-up/Disposition template Select an assessment from the grid. In the Follow-up section, select the follow up details for the patient. Note: To enter a custom reason, select the blank row and enter the details. In the Disposition section, select the desposition details. Note: To enter a custom reason, select the blank row and enter the details. Select Place Order. Document Follow-up Details
You can enter follow-up appointment and disposition details on the Follow-up/Disposition template. Open NextGen® Enterprise EHR. Open the SOAP template. In the Assessment / Plan panel, select the Follow Up button.The Follow-up/Disposition template opens. Follow-up/Disposition template The Follow-up/Disposition template is part of a group of templates that use the last assessment or diagnosis selected on any of the templates in the group as the working assessment. The application defaults to the current working assessment. If required, you can select another assessment, which becomes the new working assessment that carries forward to the other templates in the group. Do one of the following: Select the diagnosis in the grid. In the Follow-up section, select follow up along with other details and the suggested time for the follow-up visit. Open the Follow-up or Disposition Template
You can document the follow-up details for a patient using the Follow-up/Disposition template. Open NextGen® Enterprise EHR. Select a patient record. To open the Follow up/Disposition template, do one of the following: On the Intake tab, expand the Orders panel, and then select Follow Up. On the Homepage tab, expand the Orders panel, and then select Follow Up. On the SOAP tab, expand the Assessment / Plan panel, and then select Follow Up. Enter Assessment Plan Details
Open NextGen® Enterprise EHR. On the Assessment Plan Details template, select the A/P Details tab. To add or change a diagnosis in the Today’s Assessments grid, select Add or Edit to open the Add or Update Assessment template. To change the current working assessment, in the Today's Assessments grid, select the diagnosis being addressed for this encounter. The selected diagnosis appears in the Selected Dx field. It becomes the new working assessment that carries forward to the other templates that share the assessment. Enter details manually or use My Phrases or Common Phrases for the impression, differential diagnosis, and patient and provider plans in the respective sections. To enter follow-up appointment and disposition information, select Follow Up. To document counseling educational details and factors, select the Counseling Details link.
