template, select the specialty as
Family Practice
and the visit type as

Select an assessment from the grid.

Document an Assessment Plan
You can document patient plan and provider plan details for the patient's assessment. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. Open the Intake template, select the specialty as Family Practice and the visit type as Office Visit, and then select My Plan. Select the A/P Details tab.The Assessment Plan Details template opens. Select an assessment from the Today's Assessment grid.The existing assessment details appear in the corresponding fields. Enter the details or use My Phrases or Common Phrases for the impression, differential diagnosis, and patient and provider plans in the respective sections. To enter follow-up appointment and disposition information, select Follow Up. To document counseling educational details and factors, select the Counseling Details. Sorting Assessments
You can update the order of the assessments for a patient in the Reorder Assessments template. The first 12 line items are available for billing, providers may want to ensure that certain assessments are included in the upper 12 rows of the list. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. Open the Intake template, select the specialty as Family Practice and the visit type as Office Visit, and then select My Plan. Select the Assessments tab and then select Sort. Reorder Assessments window displaying the existing sort order and new sort order of assessments The Reorder Assessments template opens. In the Existing Sort Order grid, select the row you want to be the first in the list. That selection becomes number 1 in the New Sort Order grid. Continue to select the available assessment rows until you have them in the required order. Optional: If you select a row in error, select Undo Last. To start over, select Undo All. Document a Lab Order
Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. Open the Intake template, select the specialty as Family Practice and the visit type as Office Visit, and then select My Plan. The My Plan window opens. Select the Labs tab.The Lab Master template opens. Select an assessment in the Today's Assessments grid. Place a Follow-up or Disposition Order
Open NextGen® Enterprise EHR. Do one of the following: In the Assessment Plan panel, click the A/P Details link in the left pane. On the My Plan (PT) template, click the A/P Details tab. The Assessment Plan Details template opens. Click Follow Up.The Follow-up/Disposition template opens. Follow-up/Disposition template To add or update the working assessment, select the diagnosis in the grid. Select the follow up checkbox. Enter the details and time for the follow-up visit. In the Disposition field, select an option from the list. If the applicable option is not available in the list, click the top blank row and then type a value in the Disposition field. If the patient refused the selected disposition, select the Refused checkbox. In the Reason field, select an option from the list. In the Timeframe field, select an option from the list. Click Place Order.The fields clear so that another follow-up appointment can be entered. The follow-up appointments appear in the Assessment/Plan grid

Document an Assessment
You can filter the grids on the Assessments tab. For example, in the Billing Diagnosis History section, selecting the Show Chronic only checkbox displays diagnoses that are not acute. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. Open the Intake template, select the specialty as Family Practice and the visit type as Office Visit, and then select My Plan. The My Plan window opens. Select the Assessments tab.The Add or Update Assessment template opens. Select the Add common assessment link.The Common assessment window opens based on specialty, gender, and age of the patient. Select an assessment and then select Save & Close.The Today's Assessment grid displays the diagnoses details. If your practice requires a status for the diagnosis, you must select the status. Document Follow-up Details
You can enter follow-up appointment and disposition details on the Follow-up/Disposition template. Open NextGen® Enterprise EHR. Open the SOAP template. In the Assessment / Plan panel, select the Follow Up button.The Follow-up/Disposition template opens. Follow-up/Disposition template The Follow-up/Disposition template is part of a group of templates that use the last assessment or diagnosis selected on any of the templates in the group as the working assessment. The application defaults to the current working assessment. If required, you can select another assessment, which becomes the new working assessment that carries forward to the other templates in the group. Do one of the following: Select the diagnosis in the grid. In the Follow-up section, select follow up along with other details and the suggested time for the follow-up visit. To select a new diagnosis or modify an existing assessment for the current patient, select Add/Edit Assessment. With the correct description selected
