Document an Assessment

You can filter the grids on the Assessments tab. For example, in the Billing Diagnosis History section, selecting the Show Chronic only checkbox displays diagnoses that are not acute.

  1. Open NextGen® Enterprise EHR.
  2. Open a patient's record, and then open an encounter.
  3. Open the Intake template, select the specialty as Family Practice and the visit type as Office Visit, and then select My Plan.
    The My Plan window opens.
  4. Select the Assessments tab.
    The Add or Update Assessment template opens.
  5. Select the Add common assessment link.
    The Common assessment window opens based on specialty, gender, and age of the patient.
  6. Select an assessment and then select Save & Close.
    The Today's Assessment grid displays the diagnoses details. If your practice requires a status for the diagnosis, you must select the status.

    If a working assessment exists, it remains selected in the Today's Assessment grid.

  7. To update the working assessment, select the diagnosis in the Today's Assessment grid.
  8. Optional: On the Add or Update Assessment template, select an assessment and do the following:
    • To convert the ICD-9 codes to ICD-10 , select the ICD code.
    • To have your staff look for the diagnosis code, select the Staff to Look up diagnosis code, if enabled.
    • Enter the impression and differential diagnosis details in the respective sections.
    • To send the documented information to other staff, select Quick Task.
    • To remove an assessment, select Remove.
  9. Select Add/Update.
    The Today's Assessments grid displays the diagnoses details.
  10. Select Save & Close.
