Add or Update Assessment Template

You can open the Add or Update Assessment template using the Assessments link in the My Plan template. The Add or Update Assessment template allows you to add and update assessments. Document an Assessment Plan

You can document patient plan and provider plan details for the patient's assessment. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. Open the Intake template, select the specialty as Family Practice and the visit type as Office Visit, and then select My Plan. Select the A/P Details tab.The Assessment Plan Details template opens. Select an assessment from the Today's Assessment grid.The existing assessment details appear in the corresponding fields. Enter the details or use My Phrases or Common Phrases for the impression, differential diagnosis, and patient and provider plans in the respective sections. To enter follow-up appointment and disposition information, select Follow Up. To document counseling educational details and factors, select the Counseling Details. Enter Assessment Plan Details

Open NextGen® Enterprise EHR. On the Assessment Plan Details template, select the A/P Details tab. To add or change a diagnosis in the Today’s Assessments grid, select Add or Edit to open the Add or Update Assessment template. To change the current working assessment, in the Today's Assessments grid, select the diagnosis being addressed for this encounter. The selected diagnosis appears in the Selected Dx field. It becomes the new working assessment that carries forward to the other templates that share the assessment. Enter details manually or use My Phrases or Common Phrases for the impression, differential diagnosis, and patient and provider plans in the respective sections. Document Assessments and Plans

Open NextGen® Enterprise EHR. Open the SOAP template, and then expand the Assessment/Plan panel. To add the patient's assessment and plan, select an appropriate link from the left navigation bar, or select Add/Update. To copy assessments from a previous encounter to the current encounter, select Dx/AP History.If you have copied and edited information from a previous encounter, a label appears in the Assessment/Plan panel and applicable documents indicating that the information is copied and edited. Note: You can configure the functionality to copy assessments from previous encounters in the Copy Previous Visit Information section of the Practice Configuration template. Without the configuration, the Assessment, Diagnosis Copy Forward window opens in read-only mode when you select Dx/AP History. Documenting Assessment Plan Details

You can document your impressions and comments, differential diagnoses, and patient or provider plan details for an assessment using the A/P Details tab. You can update an assessment with the following details: Add or update an assessment in the Today's Assessment grid. Document a health promotion plan for the patient. Documenting Assessment Details

You can add or update a patient's assessment details in the Assessments tab. You can update the ICD-10 coding and 5010 standards using the patient's assessment. You can select the assessment details in the grid and submit the first 12 diagnoses for claim processing. You can add or update an assessment in the following sections: Billing Diagnosis History Clinical Problems My Favourites The Today’s Assessments grid on the Assessments tab contains the following details: First column displays the priority, considering that you can submit only the first 12 row items for claims. Second column displays the diagnosis description, diagnosis code, and if entered, the status, side, and site details. Third column displays the differential diagnoses. Note: If an HCC column appears in the grid, the practice administrator must turn on the Hierarchical Condition Category (HCC) coding functionality in the Problems module.
