Save the Documented Assessment Details

After you have completed documenting the patient's plan and associated details, there are different options for proceeding with the encounter. Open NextGen® Enterprise EHR. To document another assessment plan when you are still in the Assessment/Plan Details template, select a different diagnosis from the grid in Today's Assessment. Open the Assessments Tab

You can open the assessment tab using Add or Update Assessment template. Open NextGen® Enterprise EHR. Open the SOAP template, then in the Assessment/Plan section, select Add Assessments.The Add or Update Assessment template set opens. Optional: From the Intake or SOAP template, select the My Plan, and then select Assesments.The Add or Update Assessment template set opens. Document an Assessment

You can filter the grids on the Assessments tab. For example, in the Billing Diagnosis History section, selecting the Show Chronic only checkbox displays diagnoses that are not acute. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. Open the Intake template, select the specialty as Family Practice and the visit type as Office Visit, and then select My Plan. The My Plan window opens. Select the Assessments tab.The Add or Update Assessment template opens. Select the Add common assessment link.The Common assessment window opens based on specialty, gender, and age of the patient. Select an assessment and then select Save & Close.The Today's Assessment grid displays the diagnoses details. If your practice requires a status for the diagnosis, you must select the status. If a working assessment exists, it remains selected in the Today's Assessment grid. To update the working assessment, select the diagnosis in the Today's Assessment grid. Optional: On the Add

Documenting Assessment Details

You can add or update a patient's assessment details in the Assessments tab. You can update the ICD-10 coding and 5010 standards using the patient's assessment. You can select the assessment details in the grid and submit the first 12 diagnoses for claim processing. You can add or update an assessment in the following sections: Billing Diagnosis History Clinical Problems My Favourites The Today’s Assessments grid on the Assessments tab contains the following details: First column displays the priority, considering that you can submit only the first 12 row items for claims. Second column displays the diagnosis description, diagnosis code, and if entered, the status, side, and site details. Third column displays the differential diagnoses. Note: If an HCC column appears in the grid, the practice administrator must turn on the Hierarchical Condition Category (HCC) coding functionality in the Problems module. Open Billing ICD List

The Billing ICD List tab of the Problems module displays the details of the billing ICD codes of a patient. Open NextGen® Enterprise EHR, and select a patient from Patient Lookup window. Select an existing encounter from the Encounter History or create an encounter. To open the Problems module, select Problems on the Quick Buttons panel.The Problems module opens displaying the Problem List tab by default.
