Problems Module

The Problems module enables you to list and maintain specific medical problems and diagnosis information related to each patient. When you use the Problems module to add medical problems to the patient's medical record, they are categorized as either active or resolved. All of the problems added to a patient's medical record appear under the Encounter History tab. From the Problems module, you can: Set a specific date for the onset of a problem. Provide a description and fully specified name of a problem. Assign a concept ID and a billing code. Indicate whether a problem is chronic. Specify when, how, and by whom a problem was resolved. Search for all providers within the enterprise to assign the responsible provider. The Problems module has two tabs, the Problem List tab and the Billing ICD List tab. Each tab has a data display area and a data entry area, and each tab has its own features and functions for both areas. Note: If the instructions reference problems and you are using the

Specify a Diagnosis Status

In the Billing ICD List tab, you can only change the status of a diagnosis to Resolved. After you Resolve a diagnosis, you cannot change it. You can, however, duplicate (Recur) the ICD code to create an exact copy of the diagnosis with a status of Unresolved. Select a patient. Select the patient's encounter that contains the problem to specify a status. Open Problems module. Select the Billing ICD List tab. Select the problem in the Billing ICD List data entry area. Note: If there is more than one instance of a Diagnosis/Billing ICD code, select the top level displayed for the Billing ICD code to resolve all of the listed instances or select a specific Onset Date of the diagnosis to resolve the particular instance. If you are resolving multiple instances of the same Billing ICD Code, a warning message appears. Select Yes. Do one of the following: Right-click and select Resolved from the context menu. Delete an ICD Code from the ICD Billing List

You can delete an ICD code from the Billing ICD list if the Billing ICD code is not assigned to a locked encounter. Select the patient's encounter that contains the problem to modify. Open Problems module. Select the Billing ICD List tab. Do one of the following: Right-click and select Delete from the context menu. Select the Delete button on the data entry area toolbar. The Delete Diagnosis message prompts you to confirm deletion. Select Yes to delete the ICD code. Note: All deleted billing ICD entries are displayed in the Deleted Items window. Add a Problem to the Billing ICD List

From the problem list, you can add a problem to the billing ICD list. Open the Problems Module. Select a problem from the problem list. Do one of the following: Right-click the problem, and then select Add to Billing ICD List. On the date entry grid tracking toolbar, select Add to Billing ICD List. The Billing ICD Search window opens. The application maps the problem SNOMED Concept ID code to the billing ICD code and displays the results in the Billing ICD List tab. Select Mapped ICD Code Note: The Billing ICD Search window displays synonym descriptions associated with an ICD code in the search results. If the selected problem has multiple billing ICD-10 codes, the application displays the list of mapped codes. Select which billing ICD-10 code to add to the billing ICD list for the current encounter. If the selected problem has only one billing ICD-10 code, that code is automatically added to the billing ICD list for the current encounter. Select the Preferences button to display a lis

Modify Locked Encounter Diagnoses in the Billing ICD List

Locked encounters cannot be modified. However, you can duplicate (Recur) a problem and modify the duplicate. You can only Recur a problem in the Billing ICD List tab. For details on locking and unlocking an encounter, go to NextGen Healthcare Success Community, and download the latest NextGen® Enterprise User Guide. Select the patient's encounter that contains the diagnosis to modify. Open Problems module. Select the Billing ICD List tab. Select the diagnosis. Select the Recur button. NextGen® Enterprise enters the same problem with the current date. Make changes to the status and severity of the problem. Enter additional notes. Delete a Problem in the Problem List

Open the Problems Module. Select the patient's encounter that contains the problem you want to delete. Note: You can delete a problem from a patient's problem list if the problem is not assigned to a locked encounter. Select the problem in the problem list. Do one of the following: Right-click the problem, and then select Delete. On the problem list data entry area toolbar, select Delete. The Delete Problem message prompts you to confirm if you want to delete the problem. Select Yes to delete the problem. If the problem was not entered in error, clear the Entered in error check box. Select Refresh in the problem list toolbar to view the updated problem list. Note: All deleted problem list entries are displayed in the Deleted Items window.
