Document Assessments and Plans
- Open NextGen® Enterprise EHR.
- Open the SOAP template, and then expand the Assessment/Plan panel.
- To add the patient's assessment and plan, select an appropriate link from the left navigation bar, or select Add/Update.
- To copy assessments from a previous encounter to the current encounter, select Dx/AP History.
If you have copied and edited information from a previous encounter, a label appears in the Assessment/Plan panel and applicable documents indicating that the information is copied and edited.Note: You can configure the functionality to copy assessments from previous encounters in the Copy Previous Visit Information section of the Practice Configuration template. Without the configuration, the Assessment, Diagnosis Copy Forward window opens in read-only mode when you select Dx/AP History. - Optional: To manually sort the assessments according to your requirements, select Sort.
- If applicable, select the Include all Title X contraception information in the document with patient permission checkbox.
- If applicable, select appropriate values from Reason for no birth control method at end of visit, Contraception provision method, and Birth control method (BCM) at end of visit.
- To document the details of the patient's consent, select the Consent link.
Note: To create a consent note with the documented information, you can select Consent.
- To view today's assessment and procedure scheduling details or place a procedure order, select Procedure Scheduling.
- To place an order for a follow-up visit, select Follow Up.
- To refresh the information in the grid, select Refresh.
- To create an Advanced Beneficiary Notice of Non-Coverage (ABN), select ABN.
Note: The ABN button appears only if you configure the medical necessity for your state in the Copy Previous Visit Information section of the Practice Configuration template.

You can add patient assessments and plan details in the Assessment / Plan panel. - Update an Assessment on the SOAP Template
Providers can edit information on an assessment and a plan on the SOAP Plus template. - Remove an Assessment on the SOAP Template
You can remove assessments when required. - Copy Assessments from Previous Encounters
You can copy assessments from previous encounters of a patient to the current encounter instead of manually adding them. - Add or Update Assessment Template
You can open the Add or Update Assessment template using the Assessments link in the My Plan template. - Configure Title X in Assessment / Plan
You can document the birth control method (BCM) in the Assessment / Plan panel for those practices that report on Title X 2.0. - Document the Birth Control Method Details

You can place lab orders from the Labs tab using the Lab Master template or the Orders console. The Process and Submit Lab/Radiology Orders template shares information with the Lab Master template and the Orders console. - Place an Order for Diagnostic Studies
You can place a diagnostic study order with Bypass Order Module Processing enabled. - Documenting Referral Orders
You can place or manage orders for referrals of other specialists, locations, therapies, and durable medical equipment (DME) in the Referrals Orders or Referral Management template. - Document Office Procedures
You can place orders using the Office Procedures template. - Send Quick Tasks
You can send referrals to other staff members for any further action by using Quick Task. - Generate a Consent Note
You can generate a consent note.
